Immunologie of endometriosis and immunotherapy

Abstract
Endometriosis is a common gynecologic disorder characterized by the presence of endometrial tissue outside the uterine cavity. No single theory can explain all cases of endometriosis. But, Sampson's retrograde menstruation theory has gained the widest acceptance (1). This theory proposes that some endometrial tissue is refluxed through the tubes during menstruation. The viable endometrial tissue then implants and grow on peritoneal surface. This results in endometriosis. Although retrograde menstruation can occur in many women, the prevalance of endometriosis is 6,2-8,2% (2,3). It is still unknown why endometriosis develops in some women, but not in others.There are two possible initial steps in the development of endometriosis. First, endometrial cells attch on the peritoneal surface, and then invasion occurs into the mesothelium. After these steps, inflammatory cells are recruited, angiogenesis occurs and the endometrial cells start to proliferate. Although, the endometriotic tissue with its local hormonal environment influences each of these steps, immune cells and inflammatory cytokines and environmental factors also play a role.

Endometriozis, endometriyum dokusunun uterus dışında rastlanması ile karakterize olan ve sık rastlanılan bir jinekolojik hastalıktır. Bu hastalığın etiopatogenezini açıklamak için birçok teori ileri sürülmüştür. Fakat hiçbir teori tek başına bu hastalığın oluşumunu açıklayamamaktadır. En çok kabul gören teori ise, Sampson'nın retrograd menstrüasyon teorisidir (1). Bu teoriye göre, menstrüasyon sırasında akımın bir kısmı geriye doğru olur. Fallop tüplerinden periton boşluğuna gelen endometriyal doku parçalarının bir kısmı burada yapışarak büyümeye başlarlar. Bu da endometriozisi oluşturur. Buna karşın, retrograd menstrüasyon birçok kadında gözükse de endometriozis % 6.2 ile % 8.2'lik prevalansı vardır (2,3). Bu açıdan, endometriozis niye bazı kadınlarda gelişirken, diğerlerinde neden gelişmediği halen tam olarak bilinmemektedir.Endometriozis gelişimindeki olası ilk iki basamak, endometriyal hücrelerin periton yüzeyine yapışması ve ardından bu hücrelerin mezotele invazyonudur. Bu basamaklardan sonra, enflamatuar hücreler toplanır, anjiogenezis olur ve endometriyal hücreler çoğalmaya başlar. Endometriyotik dokunun lokal hormonal hali tüm bu basamakları etkilese de, bağışıklık sistemi hücreleri, enflamatuar sitokinler ve ayrıca çevresel faktörler de bu olaylarda rol oynarlar.
