The role of cytology, histology, colposcopy and human papillomavirus in evaluation of precancerous lesion of cervix

Abstract
OBJECTIVE: To determine the importance of the repeat smear, the biopsy having been directed by colposcopy and HPV test for the treatment and follow up of the patients with pathological smear results.STUDY DESIGN: Among 2294 patients, 71 patients with pathological smear results were taken into the study groups betweeen February 3, 2002 and January 1, 2004 on the extent of cytological screening in our clinic. Colposcopy, repeat smear, punch biopsi, and endocervical curettage were carried out on each patient. In this group, by comparing the repeat smear with the biopsy results, repeat smear's sensitivity, spesifisity, positive and negative predictive values were computed. Among 71 patients, 47 patients to whom we performed HPV testing formed the second study group. The HPV percentage was computed according to lesions in this group.RESULT(s): In ASCUS group; 7 mild displasia (12.9), 5 moderate/severe displasia (% 9.2) and in LSIL group 4 mild displasia (% 40), 3 moderate/severe displasia and more serious lesion (% 30) were found. The sensitivity, specificity, negative and positive predictive values of repeat smear for mild displasia were respectively, % 72.7, % 83.7, % 66.7, % 87.2 and the same proportions for moderate/severe displasia and more serious lesions were found as % 90, % 83.7, % 52.7, % 97.6. The proportion of the total HPV with moderate/high risk was found as % 17.02 in the group which we performed HPV testing. Positive HPV with moderate/high risk was found in all of the patients with HSIL. When HPV ratios were compared according to the results of biopsy; HPV was found positive among 4 of the 10 patients having mild displasia (% 40), 3 of the 5 patients having moderate/severe displasia and more serious lesion (% 60).CONCLUSION: According to our research;to follow up the patients with repeat smears who have suspicious smear results must be limited only in the patients who are congenial and coming the visits regularly. Colposcopy and taking biopsy, if necessary, must be the first preference because these patients may have high grade lesion to high extent. HPV screening can be used as an aided test in the follow up of these patients.

AMAÇ: Patolojik smear sonucu ile gelen hastaların tedavi ve takibinin yönlendirilmesinde smear tekrarının, kolposkopi ile yönlendirilmiş biopsinin ve HPV testinin önemini belirlemek.MATERYAL ve METOD: Kliniğimizde uygulanan sitolojik tarama kapsamında, 3/2/2002-26/1/2004 tarihleri arasında serviko-vaginal smear alınan 2294 hasta içerisinde patolojik smear sonuçları ile gelen 71 hasta çalışma grubuna alındı. Hastaların hepsine smear tekrarı, kolposkopi, yönlendirilmiş punch biopsi ve endoservikal kürtaj yapıldı. Bu grupta smear tekrarı biopsi sonuçları ile karşılaştırılarak smear tekrarının sensitivitesi, spesifisitesi, pozitif ve negatif prediktif değerleri hesaplandı. Çalışmanın ikinci grubunu 71 hasta içinde HPV bakılan 47 hasta oluşturdu. Bu grup içinde lezyonlara göre HPV yüzdeleri hesaplandı.BULGULAR: ASCUS grubunda 7 hafif displazi (% 12.9), 5 orta/ağır displazi (% 9.2) ve LSIL grubunda 4 hafif displazi (% 40), 3 orta/ağır displazi ve daha ağır lezyon (% 30) saptandı. Smear tekrarının hafif displazi için sensitivite, spesifisite, negatif ve pozitif prediktif değerleri sırasıyla % 72.7, % 83.7, % 66.7, % 87.2 ve aynı oranlar orta/ağır displazi ve daha ağır lezyonlar için % 90, % 83.7, % 52.7, % 97.6 olarak bulundu. HPV bakılan grupta toplam orta/yüksek riskli HPV oranı % 17.02 olarak bulundu. HSIL bulunan 2 hastanın 2'sinde de orta/yüksek riskli HPV pozitif bulundu. Biyopsi sonuçlarına göre HPV oranları karşılaştırıldığında, hafif displazi bulunan 10 hastanın 4'ünde (% 40), orta ağır displazi ve daha ağır lezyon bulunan 5 hastanın 3'ünde HPV pozitif bulundu (% 60).SONUÇ: Çalışmamız sonuçlarına göre, şüpheli smear sonucu olan hastalarda smear tekrarı ile takip, sadece uyumlu ve düzenli kontrollere gelebilecek hastalarla sınırlandırılmalıdır. Bu hastalarda oldukça önemli oranda yüksek grade lezyon bulunabileceğinden dolayı kolposkopi ve gereğinde biopsi alınması öncelikli tercih olmalıdır. HPV taraması, bu hastaların takibinde smeare yardımcı bir test olarak kullanılabilir.
