Serum androgen levels in preeclampsia

Abstract
OBJECTIVE: The purpose of the study was to determine whether maternal serum levels of androgens, especially testosterone, are higher in patients with preeclampsia than in matched normotensive control subjects.STUDY DESIGN: Serum testosterone, dehydroepiandrosterone sulfate, sex hormone binding globulin, and estradiol levels were measured in 101 subjects in the third trimester of pregnancy with documented preeclampsia and 90 healthy, normotensive women with similar maternal and gestational ages. The sexes of the babies were recorded.RESULTS: Total testosterone and free testosterone levels were significantly higher in patients with preeclampsia (1.50±1.60 ng/ml, and 3.51±2.36 pg/ml respectively), than in the control group (0.80±065 ng/ml and 1.77±1.06 pg/ml respectively) (p0.05). There were also no significant differences in maternal age, gestational age, body mass index, and neonatal sex.CONCLUSION: Levels of the potent androgen testosterone were significantly higher in primigravid women with preeclampsia than in normotensive women with similar gestational and maternal ages. This difference may indicate a role for testosterone in the pathogenesis of preeclampsia.

AMAÇ: Preeklampsi tanısı koyulan gebeler ile normotansif gebelerin serum androjen seviyelerini karşılaştırmak, testosteronun preeklampsi ile ilişkisini değerlendirmektir.MATERYAL ve METOD: Kliniğimizde takip ve tedavi edilen, nullipar ve multipar 101 preeklamptik gebeden grup I ve 90 normotansif gebeden grup II oluşturularak, toplam 191 gebe üçüncü trimesterinde prospektif olarak çalışma kapsamına alındı. Çoğul gebelikler ile mort fetusu olan gebeler çalışma dışı bırakıldı. Doğan bebeklerin cinsiyeti kaydedildi. Tüm olgulardan venöz kan alınarak, total testosteron, serbest testosteron, estradiol, DHEAS, SHBG seviyelerine bakıldı. İstatistiksel analizlerde Chi-Square ve Student-T testleri kullanıldı. p 0.05).SONUÇ: Preeklamptik grupta total ve serbest testosteron düzeylerinin, aynı gebelik haftasındaki normotansif gebelerden daha yüksek bulunması, henüz etyopatogenezi net olarak anlaşılmamış preeklampsinin ortaya çıkmasında androjenlerin predispozan bir faktör olabilecegini düşündürmektedir. Fetus cinsiyetinin androjen düzeyi üzerinde önemli rol oynamadığı tespit edildi.
