Abstract

OBJECTIVE: The aim of this study is to analyze and define the possible causes of maternal deaths in Council of Forensic Medicine Morgue Department.STUDY DESIGN: In this study postmortem examination of 63 maternal deaths were evaluated in Council of Forensic Medicine Morgue Department in a time period of 1998-2003. Age distribution, duration of pregnancy, postnatal period, pathological alterations of visceral organs and causes of death were investigated.RESULT(s): Age distribution of these 63 cases was as follows: 5 cases younger than 20 years of age (% 8); 28 cases age group 21-30 (% 44.4) and 30 cases elder than 30 years of age (% 47.6). Average age is found as 30.9 ±6.7 (SD). Death occured as follows: 28 cases during pregnancy (% 44.4), 8 cases (% 12.7) during delivery, 16 cases 24 hours following delivery (% 25.4), 11 cases 24 hours-42 days after delivery (% 17.5). Causes of death were linked a wide variety of complications to due pregnancy and complications related to pregnancy (% 58.7), due to previous systemic diseases those were exacerbated after pregnancy in 11 cases (% 17.5), finally due to causes independent from pregnancy in 9 cases (% 14.3). Only 6 cases were (% 9.5) identified as negative autopsy.CONCLUSION(s): It is a common phenomenon in forensic pathology practice that majority of maternal deaths to be found as sudden and unexpected death cases. Fortunately, in order to decrease the incidence of maternal death cases, interdisciplinary approach should be encouraged in medical practice. Moreover, understanding of the underlying causes of maternal deaths will definitely decrease the overall maternal death frequency in daily medical practice.

AMAÇ: Çalışmanın amacı, Adli Tıp Kurumu Morg İhtisas Dairesi'nde otopsileri yapılan olgularda anne ölümlerini irdelemek ve ölüm nedenlerini tanımlamaktır.MATERYAL ve METOD: Çalışmamızda 1998-2003 yılları arasında Adli Tıp Kurumu Morg İhtisas Dairesin'de otopsileri yapılan 63 anne ölüm olgusu değerlendirildi. Olguların yaşları, gebelik süresi, doğum sonrası periyod, iç organlarda saptanan patolojik değişimler ve ölüm nedenleri değerlendirildi.BULGULAR: Altmış üç olgu yaş gruplarına göre sınıflandırıldığında, 20 yaş altında 5 olgu (% 8), 21-30 yaş arasında 28 olgu (% 44.4) ve 30 yaş üstünde 30 olgu (% 47.6) saptandı. Yaş ortalaması 30.9±6.7 (SD) bulundu. Ölüm, 28 olguda (% 44.4) gebelik , 8 olguda (% 12.7) doğum sırasında, 16 olguda (% 25.4) doğum sonrası ilk 24 saatte, 11 olguda (% 17.5) doğum sonrası 24 saat-42 gün arasında gerçekleşmişti. Ölümün, 37 olguda (% 58.7) direkt gebelik ve doğumla ilişkili nedenlere, 11 olguda (% 17.5) gebelik öncesinde var olan ve gebelik döneminde şiddetlenen sistemik hastalıklar gibi indirekt nedenlere, 9 olguda (% 14.3) ise, gebelikten bağımsız diğer nedenlere bağlı olarak geliştiği görüldü. Altı olgu (% 9.5) negatif otopsi olarak kabul edildi.SONUÇ: Anne ölüm olgularının çoğu ani ve şüpheli ölüm olması nedeniyle Adli Tıp pratiğinde sıkça karşımıza çıkmaktadır. Anne ölümlerinin azaltılması amacı ile farklı tıp dallarının işbirliği içinde çalışması gereklidir. Ayrıca, otopsi ile ölüm nedeninin doğru olarak belirlenmesi de bu ölümlerin azaltılmasına katkı sağlayacaktır.
