Retrospective Evaluation of Neonates Treated for Cephalohematoma

Abstract

Objective: Cephalohematoma is subperiostal hemorrhage caused by ruptured blood vessels between the cranium and periosteum. It occurs in 0.4–2.5% of all live births and is most frequently seen in the parietal region. Fractures of the cranial bones may be detected in 5–20% of patients with cephalohematoma. The aim of this study was to highlight the clinical features, risk factors, and morbidities of neonates with cephalohematoma. Materials and Methods: This retrospective chart review included neonates born in our hospital between January 2016 and December 2018 who were found to have cephalohematoma on physical examination and underwent imaging for suspected fracture. All patients in the study were born at gestational age ≥37 weeks and birth weight ≥2000 g. Radiography was the preferred diagnostic method for patients with suspected fracture; cranial computed tomography was performed to establish a definite diagnosis if radiographs were inconclusive. Risk factors for fracture and intracranial hemorrhage were investigated. Results: Of 3486 infants born in our hospital during the study period, 87 (2.4%) met the study inclusion criteria. The patient group comprised 38 males (43.7%) and 49 females (56.3%). Mean gestational age was 38.9±1.3 weeks, mean birth weight was 3247±403 g, and mean head circumference was 36±0.3 cm. Eighty-one (93.1%) of the patients were born by normal spontaneous vaginal delivery while 6 (6.8%) were born by cesarean section. Seventy-three (83.9%) of the neonates were born to primiparous mothers, 8 patients had prolonged labor, and 6 had abnormal fetal presentation. Cranial bone fracture was detected in 24 (27.5%) patients, with 50% of fractures in the parietal bone. Intracranial hemorrhage was detected in 9 (37.5%) of the patients with fracture and was most commonly subdural hemorrhage. One patient received medical treatment during clinical follow-up and two patients underwent surgery. Conclusion: Although cephalohematoma is often a self-limiting birth trauma, clinicians should be aware of its possible association with cranial fracture or intracranial hemorrhage.

Amaç: Sefal hematom, kraniyum ile periost arasındaki damarların rüptüre olması sonucu oluşan subperiostal kanamadır. İnsidans tüm canlıdoğumlarda %0,4 ile %2,5 arasındadır. En sık pariyetal bölgede görülür. Sefal hematom saptanan hastaların %5-20’sinde kraniyal kemiklerde fraktür olabilir. Çalışmamızda fizik muayenesinde sefal hematomsaptanan, fraktür şüphesiyle ileri görüntüleme yapılan hastaların kliniközelliklerini, risk faktörlerini ve karşılaşılan morbiditeleri vurgulamayıamaçladık.Gereçler ve Yöntem: Ocak 2016 ve Aralık 2018 tarihleri arasında hastanemizde doğan, fizik muayenesinde sefal hematom saptanıp fraktür şüphesi ile ileri görüntüleme yapılan hastaların dosyaları restrospektif olarakincelendi. Çalışmaya gebelik haftası ≥ 37 hafta, doğum ağırlığı ≥ 2000g olan bebekler dahil edildi. Hastaların demografik bilgileri kaydedildi.Fraktür şüphesi olan hastalarda tanısal metot olarak öncelikle direkt grafitercih edildi. Direkt grafi ile fraktür ayrımı yapılamayan grupta bilgisayarlıbeyin tomografisi planlanarak tanı kesinleştirildi. Fraktür ve intrakraniyalkanama için olası riskler araştırıldı.Bulgular: Çalışma döneminde hastanemizde doğan toplam 3486 bebeğin dosyası incelendi. Fizik muayenesinde sefal hematom saptanıpfraktür şüphesi ile ileri görüntüleme yapılan 87 hasta (%2,4) çalışmayadahil edildi. Hastaların 38’ini kız (%43,7), 49’unu (%56,3) erkek bebekler oluşturmaktaydı. Ortalama gebelik haftası 38,9±1,3, doğum ağırlığı3247±403 g, baş çevresi ise 36±0,3 cm idi. Bebeklerin 81’i (%93,1) normal spontan vajinal yolla doğarken, 6’sının (%6,8) sezaryen ile doğduğubelirlendi. Çalışma grubundaki 73 (%83,9) annenin ilk doğumu olduğusaptandı. Sekiz hastada doğum eyleminin uzadığı, 6 hastada anormalfetal prezentasyon ile doğumun gerçekleştirildiği kaydedildi. Kraniyalkemik fraktürü 24 (%27,5) hastada tespit edildi. En sık fraktür pariyetal(%50) kemikteydi. Fraktür saptanan 9 (%37,5) hastada intrakraniyal kanama mevcuttu. En sık intrakraniyal kanama subdural kanamaydı. Klinikizlemde bir hastaya medikal tedavi başlandı, iki hastaya ise cerrahi müdahalede bulunuldu.Sonuç: Sefal hematom sıklıkla kendini sınırlandıran bir doğum travmasıolsa da kraniyal fraktürün eşlik edebileceği veya intrakraniyal kanamanınhabercisi olabileceği unutulmamalıdır.
