Does Vaginal Preparation with Povidone-Iodine Prior to Cesarean Delivery Reduce the Risk of Endometritis?

Abstract

Objective: The purpose of the present study was to determine whether vaginal preparation with povidone-iodine prior to cesarean delivery decreases the incidence of postpartum wound infection and endometritis. Material and Methods: The present study was a prospective randomized controlled trial in which subjects underwent cesarean delivery with or without prior vaginal preparation using povidone-iodine solution The primary outcome measure was the rate of postpartum wound infection and endometritis. Results: A significant decrease in post-cesarean wound was noted in the group that received the povidone-iodine vaginal preparation (n=332) compared with the control group (n=335) (p=0.015). No statistically significant differences in the incidence of endometritis were noted between the experimental and control groups among women who were not in labor at the time of the cesarean delivery (p=0.841). Conclusion: Vaginal preparation with povidone-iodine solution immediately prior to a cesarean delivery reduces the risk of post-operative wound infection.

Amaç: Sezaryen doğum öncesi povidon iyot ile vajinal hazırlığın postpartum yara enfeksiyonu ve endometrit oranlarını azaltıp azaltmadığını belirlemek.Gereç ve Yöntemler: Çalışmamız sezaryen doğum öncesi povidon iyot ile hazırlık yapılan veya yapılmayan olguları içeren randomize kontrollü bir çalışmadır. Çalışmamızın primer sonucu postpartum yara yeri enfeksiyonu ve endometrit oranlarıdır.Bulgular: Kontrol grubu (n=335) ile karşılaştırıldığında povidon iyot ile vajinal hazırlık yapılan grupta (n=332) post-operatif yara enfeksiyonu belirgin şekilde azalmıştı (p=0,015). Endometrit insidansı her iki grupta da benzerdi (p=0.841). Sonuç: Sezaryen doğum öncesi povidon iyot ile vajinal hazırlık post-opertif yara yeri enfeksiyonu riskini azaltır.
