Outcomes of Post-traumatic Stiff Elbow Arthrolysis

Abstract

Objective: In this study, the results of open release surgeries performed for the stiff elbows which were caused by intrinsic, extrinsic or by combinationof both factors were evaluated.Methods: Twenty-three elbows of 22 patients who could not perform functional elbow motions were surgically treated between January 2005 andDecember 2012. The mean age was 30.6±11.4 years (16 to 67 years), and the average follow-up period was 81 months (58 months to 12 years). Elbow arcof motions was recorded pre- and postoperatively. Patients were evaluated clinically by using quick Disabilities of the Arm, Shoulder and Hand (DASH)self-report questionnaire, and MAYO elbow performance scores.Results: The mean preoperative elbow arc of motion was 42 degrees (0-90), and it became 109 degrees (70-140) postoperatively after at least 58-monthfollow-up. The average increase was 67.8±25 degrees (30-125). The mean Quick DASH score was 18.2±12.7 (6.8 to 63.6). According to MAYO elbowrating system, one patient who had experienced infection following the initial fracture treatment had poor result, three patients had fair results, 17patients had good results, and one patient who had bilateral elbow stiffness had excellent results.Conclusion: Although good results can be achieved by open release of the stiff elbows, one must keep in mind that preventing the stiffness ratherthan solving this problem would bring better results. As 15 patients with stiff elbows in this study had a previous surgery performed for an elbow regionfracture, stabile fixation, meticulous hemostasis, shorter immobilization period and early rehabilitation can be considered as the most important steps foravoiding elbow stiffness where surgery is indicated.

Amaç: Bu çalışmada intrinsik, ekstrinsik veya kombine sebeplerden kaynaklanan sert dirsek olgularındaki artroliz sonuçları değerlendirildi. Yöntemler: Ocak 2005 ile Aralık 2012 yılları arasında fonksiyonel dirsek hareketlerini yerine getiremeyen 22 hastanın 23 dirseğine uygulanan artroliz sonuçları değerlendirildi. Ortalama yaş 30,6±11,4 yıl (16 ile 67 yaş arasında) ve ortalama takip süresi 81 aydır (58 ay ile 12 yıl arasında). Dirsek eklem hareket açıklıkları ameliyat öncesinde ve sonrasında kayıt edilmiştir. Hastalar klinik olarak hızlı Kol, Omuz ve El Sorunları (DASH) anketi ve MAYO dirsek performans skorları ile değerlendirilmiştir. Bulgular: Ameliyat öncesi ortalama dirsek hareket açıklığı 42 derece (0 ile 90 derece arasında) iken, en az 58 aylık takip süresi sonucunda ölçülen değer 109 derece (70 ile 140 derece arasında) saptanmıştır. Ortalama artış 67,8±25 derecedir (30-125). Ortalama hızlı DASH skoru 18,2±12,7’dir (6.8 ile 63.6). MAYO dirsek değerlendirme sistemine göre kırık tespiti için geçirdiği ilk ameliyatından sonra enfeksiyon gelişen bir hastada kötü sonuç, üç hastada orta sonuç, 17 hastada iyi sonuç ve bir hastanın her iki dirseğinde de mükemmel sonuç elde edilmiştir. Sonuç: Sert dirsek açık gevşetme ameliyatlarından iyi sonuçlar alınsa da, bu problemi çözmekten ziyade dirsek sertliğini önlemenin daha iyi olacağını unutmamak gerekir. Bu çalışmadaki 23 dirseğin 15’inin daha önce dirsek bölgesi kırığı geçirmiş olduğunu göz önüne alarak, kırıklardan sonra stabil tespit, çok özenli kanama kontrolü sağlanması, kısa immobilizasyon ve erken rehabilitasyonun dirsek sertliğini önlemede en önemli basamaklar olduğunu hatırlamak gerekir.
