Our Parenchyma-Sparing Surgical Treatment Results in Carcinoid Tumors and Changes in the Surgical Approach

Abstract
Objective: Hilar/mediastinal lymph node sampling together with lobectomy is the most common surgical method used for the surgical treatment of carcinoid tumors. Bronchoplastic approaches together with lobectomy enable sparing of normal lung tissue and provide an alternative surgical approach to avoid pneumonectomy by allowing sleeve resection to be performed for centrally located tumors. The aim of this study was to present our experiences with parenchyma-sparing open surgical treatment in the carcinoid tumor cases performed in our clinic in light of the new developments in the literature. Method: Eleven cases with tumor that had been diagnosed as carcinoid tumor and undergone surgical resection at our clinic between 2006 and 2012 were retrospectively investigated for tumor epidemiology, diagnostic methods, tumor localization, surgical treatment method, type and stage of the tumor and postoperative complications. Results: The study population with a mean age of 54.81±13.75 years (31-72 years) consisted of 7 male and 4 female patients. Cough was the most common presentation symptom with 8 cases (72.7%). A bronchoscopic biopsy was used for the definite diagnosis in 8 cases (72.7%). Eleven (84.6%) typical and 2 (15.4%) atypical carcinoid tumors were detected in a total of 13 cases. The most common surgical methods used were lobectomy in 7 (53.8%) (3 sleeve, 1 bronchoplastic) and wedge resection in 4 cases. We found bilateral synchronous and 3 years later metachronous carcinoid tumor in one case. The postoperative pathological diagnosis was T2 N0 M0in 6 (54.5%) and T1 N0 M0in 3 cases (27.2%). During postoperative period 1 case of atelectasis and 1 case with prolonged air leak were observed. Conclusion: Although we still use thoracotomy and lobectomy for the surgical treatment of carcinoid tumors in our clinic, we currently tend to perform minimal invasive surgery as VATS and robotic surgery techniques that are gradually becoming more popular.

Amaç: Karsinoid tümörlerin cerrahi tedavisinde lobektomi ile birlikte yapılan hiler/mediastinal lenf noduörneklemeleri en sık uygulanan cerrahi yöntemlerdir. Lobektomi ile beraber yapılan bronkoplastik yaklaşımlar ise hem normal akciğer dokusunun korunmasını hem de santral yerleşimli tümörlerde sleeverezeksiyona olanak vererek pnömonektomiden kaçınmada alternatif cerrahi yaklaşım sağlamaktadır. Buçalışmanın amacı, kliniğimizde opere edilen karsinoid tümörlü olgularda uyguladığımız parankim koruyucu açık cerrahi tedavi deneyimlerimizi literatürde yer alan cerrahi yenilikler ışığında sunmaktır.Yöntem: Kliniğimizde 2006-2012 yılları arasında karsinoid tümör tanısı almış ve cerrahi rezeksiyonuygulanmış 11 olgu tümörün epidemiyolojisi, tanı yöntemleri, tümörün lokalizasyonu, cerrahi tedaviyöntemi, tipi ve evresi ile postoperatif komplikasyonlar açısından geriye dönük olarak incelendi.Bulgular: Hastaların yaş ortalaması 54.81±13.75 yıl (31-72 yıl) olup, 7’si erkek, 4’ü kadındı. Öksürük8 olguyla (%72.7) en sık gözlenen başvuru yakınmasıydı. Olguların 8’inde (%72.7) kesin tanı bronkoskopik biyopsi ile kondu. On bir olguda 11’i tipik (%84.6), 2’si atipik (%15.4) olmak üzere toplam 13karsinoid tümöre rastlandı. En sık uygulanan cerrahi teknikler, 3’ü sleeve, 1’i bronkioanjioplastik olmaküzere 7 lobektomi (%53.8) ve 4 olguda wedge rezeksiyondu. Yalnızca tek olguda bilateral senkron ve3 yıl sonra metakron karsinoid tümör saptandı. Postoperatif patolojik tanı olarak en sık 6 olguda(%54,5) T2N0M0, 3 olguda ise (%27.2) T1N0M0 saptandı. Postoperatif dönemde 1 olguda atelektazi 1olguda ise uzamış hava kaçağı gözlendi.Sonuç: Kliniğimizde karsinoid tümörlü hastaların cerrahi tedavisinde her ne kadar hâlen torakotomi ilelobektomi uygulansa da son zamanlarda kullanımları gittikçe artan VATS ve robotik cerrahi teknikleredoğru yönelim olmuştur.
