Abstract

Introduction: Acute appendicitis (AA) is one of the most commonly reported surgical diseases in emergency deparment (ED). Discussions are still ongoing on the importance of laboratory tests in the diagnosis of AA in ED and in the differentiation of complicated and noncomplicated appendicitis. This study aims to investigate the usefulness of neutrophil/lymphocyte ratio (NLR), monocyte/lymphocyte ratio(MLR), platelet/lymphocyte ratio (PLR), mean platelet volume/platelet count (MPV/PC) ratio indetecting AA and its importance in differentiating complicated and noncomplicated appendicitis. Material and Method: 528 patients who admitted to the ED with abdominal pain and performed appendectomy between January 2017 and January 2020 were retrospectively evaluated. 474 of these patients were included in the study. The study group was divided into two groups as complicated and noncomplicated appendicitis according to their postoperative histopathology. The age, gender, NLR, MLR, PLR, MPV/PC ratio sand Creactive protein (CRP) values of the patients at the time of admit to the ED, hospitalization time, imaging reports and surgery reports were compared between the groups. Giriş: Akut apandisit (AA), acil servislerde en sık görülen cerrahi hastalıklardandır. Acil servislerde AA tanısının konulmasında, komplike ve komplike olmayan apandisit ayrımında laboratuvar testlerinin önemi konusundaki tartışmalar halen devam etmektedir. Bu çalışma, nötrofil/lenfosit oranı (NLR), monosit/lenfosit oranı (MLR), trombosit/lenfosit oranı (PLR), ortalama trombosit hacmi/trombosit sayısı oranının (MPV/PC)AA’yı tespit etmede yararlılığını ve komplike ve komplike olmayan apandisiti ayırt etmede önemini araştırmayı amaçlamaktadır. Gereç ve Yöntem: Ocak 2017-Ocak 2020 tarihleri arasında karın ağrısı ile acil servise başvurup appendektomi yapılan 528 hasta retrospektif olarak tarandı. Bu hastaların 474’ü çalışmaya dahil edildi. Çalışma grubu postoperatif histopatolojisine göre komplike ve komplike olmayan apandisit grubu olarak ikiye ayrıldı. Gruplar arasında hastaların yaş, cinsiyet, acil servise başvuru anında NLR, MLR, PLR, MPV / PC oranları ve C-reaktif protein (CRP) değerleri, hastanede yatış süresi, görüntüleme raporları ve ameliyat raporları karşılaştırıldı.
