CLINICAL AND SURGICAL EVALUATION OF SPINAL EPENDYMOMAS

Abstract

Introduction:Spinal tumors are located 55% extradural, 40% intradural extra medullary, and 5% intradural intramedullary. Spinal ependymomas are very rare, low-grade tumors, mostly located intradural intramedullary. The most common intramedullary spinal tumor in adults is ependymoma. Due to the excess of ependymal cells, more than half of them are seen in the filum terminale. Material and Method:Eleven patients who were operated on with the diagnosis of spinal tumor at our clinic and whose pathological result was ependymoma between 2016 and 2021 were evaluated retrospectively in terms of demographic characteristics, admission complaints, preoperative neurological examination findings, locationand size of the mass, and postoperative complications. Results:The mean age of the patients was 52.7 years (range: 27-68 years). Nine of them were female and two were male. The most of the complaints were low back pain, pain and numbness at the extremities. The tumors were located in the filum terminale region in seven patients, in the cervical region in three patients, in the thoracic region in one patient. All patients underwent surgery, and after laminectomy, the dura was opened and gross total tumor excision was performed using microsurgical technique. All interventions were performed with OPMI and neuromonitor. In the postoperative period, seizures occured in one patient who was previously known to have epilepsy, wound leakage occured in one patient. Conclusion:Gross total excision is the main target in the surgery of spinal ependymomas. The use of OPMI and neuromonitor reduces the risk of surgical complications, and good neurological outcomes are achieved after gross total excision

Özet

Giriş: Tüm spinal tümörlerin %55’i ekstradural, %40’ı intradural ekstramedüller ve %5’i intradural intramedüller yerleşimlidir. Spinal ependimomlar çoğunlukla intradural intramedüller yerleşimli, oldukça nadir görülen, düşük dereceli tümörlerdir. Erişkinlerde en sık intramedüller spinal tümör ependimomlardır. Ependimal hücrelerin fazlalığı nedeniyle yarısından çoğu filum terminalede görülür. Gereç ve Yöntem: 2016-2021 yılları arası kliniğimizde spinal tümör tanısıyla yatırılarak opere edilen ve patoloji sonucu ependimom olarak gelen 11 hasta retrospektif olarak demografik özellikler, başvuru şikayetleri, preoperatif nörolojik muayene bulguları, kitlenin yerleşim yeri, boyutu ve postoperatif komplikasyonlar açısından değerlendirildi. Bulgular: Hastaların yaş aralığı 27-68olup ortalama yaşı 52.7 idi. Hastaların 9’u kadın, 2’sierkekti. En sık başvuru şikayetleri, bel ağrısı, ekstremitelerde ağrı ve uyuşmaydı. 7 hastada filum terminale bölgesinde, 3 hastada servikal bölgede, 1 hastada da torakal bölgede yerleşimli tümör mevcuttu. Tüm hastalar cerrahiye alınarak prone pozisyonda tutulan bölgeye laminektomi sonrası dura açılarak mikroşirürjikal teknikle grosstotal tümör eksizyonu uygulandı. Tüm girişimler OPMİ ve nöromonitör eşliğinde gerçekleştirildi. Postoperatif dönemde daha önce epilepsi hastası olduğu bilinen bir hastada nöbet ve bir hastada da yara yeri akıntısı oldu. Sonuç: Genellikle intramedüller yerleşim gösteren spinalependimomların cerrahisinde hedef gross total eksizyondur. OPMİ ve nöromonitör kullanımı cerrahi komplikasyon riskini azaltmakla birlikte gross total eksizyon halinde iyi sonuçlar alınmaktadır.
