COMPARISON OF RADIOLOGICAL RESULTS OF SHORT AND LONG SEGMENT POSTERIOR FIXATION IN INSTABIL THORACOLOMBER JUNCTION BURST FRACTURES

Abstract

Introduction: Unstable fractures of the thoracolumbar spine are most commonly treated with posterior pedicle screw fixation. However, the issue of whether short segment fixation is sufficient in the treatment of these patients is still controversial. In our study, our aim was to compare the radiological results of short segment fixation (SSF) with pedicle screw applied to the fractured vertebra and traditional long segment fixation (USF). Materials and Methods: 21 patients who were hospitalized in Manisa Alaşehir State Hospital between 2016- 2019 and İzmir Katip Çelebi University (İKÇÜ) Atatürk Training and Research Hospital Orthopedics and Traumatology Clinic between 2019-2020 and underwent surgical treatment due to unstable vertebral burst fracture were included in the study. The patients were divided into two groups according to the number of instrumented level. Group 1 included 12 patients treated by SSF (six-screw construct including the fracture level). Group 2 included 9 patients treated by LSF (eight-screw construct excluding the fracture level). The radiological images of the patients were scanned retrospectively, and local kyphosis angles (LKA), ratio of anterior body height (ABH) to posterior body height (PBH) and depression degrees (DD) were evaluated. Results: Patients in both groups were similar in terms of age, gender, trauma etiology, fracture level and type, length of hospital stay and follow-up, preoperative LKA, ABH/PBH ratio, and DD (p˃0.05). There was no significant difference between the two groups in terms of the amount of postoperative LKA correction and the sustainability of LKA at the last follow-up (p˃0.05). A significant difference in favor of SSF was found between the two groups in the ratio of ABH/PBH both after surgery and at the last follow-up (p˂0.05). Conclusion: Short segment posterior fixation, including fracture level, results in kyphosis correction and preservation of sagittal correction, similar to long segment posterior fixation for thoracolumbar junction fractures. In addition, with the high number of moving segments, it provides superiority to long segment fixation.

Özet

Giriş: Torakolomber omurga instabil kırıkları en sık posterior pedikül vidası fiksasyonu ile tedavi edilir. Ancak bu hastaların tedavisinde kısa segment fiksasyonun yeterli olduğu konusu hala tartışmalıdır. Çalışmamızda amacımız kırık vertebraya da pedikül vidası uygulanan kısa segment fiksasyon (KSF) ile geleneksel uzun segment fiksasyonu (USF) radyolojik sonuçlarıyla karşılaştırmaktı. Gereç ve Yöntem: 2016-2019 yılları arasında Manisa Alaşehir Devlet Hastanesi ve 2019-2020 yılları arasında İzmir Katip Çelebi Üniversitesi (İKÇÜ) Atatürk Eğitim ve Araştırma Hastanesi Ortopedi ve Travmatoloji Kliniği’ne instabil vertebra burst kırığı nedeniyle yatan ve cerrahi tedavi yapılan 21 hasta çalışmaya dahil edildi. Hastalar tespit edilen vertebra sayısına göre iki gruba ayrıldı. Birinci grup, KSF (kırık seviyesi dahil altı vidalı tespit) ile tedavi edilen 12 hastayı içeriyordu. İkinci grup, USF ile tedavi edilen 9 hastayı içeriyordu (kırık seviyesi hariç sekiz vidalı tespit). Hastaların radyolojik görüntüleri retrospektif taranarak lokal kifoz açıları (LKA), anterior korteks yüksekliğinin (AKY) posterior korteks yüksekliğine (PKY) oranı ve çökme dereceleri (ÇD) değerlendirildi. Bulgular: Her iki gruptaki hastalar yaş, cinsiyet, travma etiyolojisi, kırık seviyesi ve tipi, hastanede yatış süresi ve takip süreleri, ameliyat öncesi LKA, AKY/PKY oranı ve ÇD açısından benzerdi (p˃0,05). Ameliyat sonrası LKA düzeltme miktarı ve son takiplerde LKA’nın sürdürülebilirliği açısından iki grup arasında anlamlı fark yoktu (p˃0,05). AKY/PKY oranında ise hem ameliyat sonrası hem de son takiplerde KSF lehine anlamlı fark bulundu (p˂0,05). Sonuç: Kırık seviyesinin de dahil edildiği kısa segment posterior fiksasyon, torakolomber bileşke kırıkları için uzun segment posterior fiksasyona benzer şekilde kifoz düzeltilmesini ve sagittal korreksiyonun korunmasını sağlamaktadır. Ayrıca hareketli segment sayısının fazla olmasıyla uzun segment fiksasyona üstünlük göstermektedir.
