The corelation between retinopathy and diastolic dysfunction in type Iı diabetes mellitus

Abstract
One of the complications that is associated with diabetes is the heart disease. There are studies being published about development of congestive heart fail-ure in the diabetic patient group who doesn't have any ishemic, valvuler or hyper-sensitive heart disease. The importance of left ventricular systolic dysfunction in the course of congestive heart failure is well known. But in the diabetic patient group, left ventricular Filling abnormalities can be established before systolic dysfunction occurs. This study was designed for to demonstrate the Tilling abnomalities in the course of the type 2 diabetes mellitus. The left ventricular functions of SO type 2 diabetic and 27 healthy patients were studied with doppler ecocardiography and the re-lationship of diastolic dysfuntion with retinopathy was investigated. At the end of the study, diastolic function abnormalities was discovered with nor-mal systolic function and diastolic dysfunction was more apparent in the group that had retinopathy. Because the duration of diabetes was longer, fasting blood sugar and heart rate were higher in the patient group that had retinopathy, it was thought that this situation could be explained with the microangiopathic changes or autonomic dysfunction.

Diabete bağlı olarak gelişen komplikasyonlardan biri de kalp hastalığıdır. İskemik, valvuler ya da hipertansif kalp hastalığı olmaksızın, diabetin seyrinde konjestif kalp yetmezliği görülebileceğine dair çalışmalar yayınlanmaktadır. Konjestif kalp yetmezliği gelişiminde sol ventrikül sistolik fonksiyon bozukluğunun önemi bellidir. Diabetli hastalarda ise, sistolik fonksiyon bozulmadan, daha erken dönemde sol ventrikül doluş bozuklukları saptanabilir. Bu çalışmada da Tip 2 diabetes mellitus seyrinde doluş bozukluklarını saptamak amacı ile planlanmış 50 Tip 2 diabetes mellituslu ve 27 sağlıklı olgunun sol ven-trikül fonksiyonları Doppler ekokardiyografi ile incelenip, diastolik fonksiyonun retinopati ile ilişkisi araştırılmıştır. Çalışmanın sonucunda, Tip 2 diabetli hastalarda sistolik fonksiyon normal olduğu halde, diastolik fonksiyon bozuklukları saptandı ve retinopati olan grupta diastolik fonksiyon bozukluğunun daha belirgin olduğu saptandı. Retinopatisi olan diabetik hasta grubunda, hastalığın süresi daha uzun, kan şekeri ile kalp hızının daha yüksek olması nedeniyle, bu durumun diabetin kötü progresyonu, diabete bağlı mikroanjiopatik değişiklikler ya da otonom disfonksiyon ile açıklanabileceği düşünüldü.
