Closed reduction and hip spica cast with K-wire application for pediatric femoral fractures

Abstract
bjectives: The aim is to evaluate the results of closed Oreduction and incorporated hip spica cast application for the femoral diaphyseal fractures of pediatric patients. Methods: 48 Pediatric patients with femur diaphyseal fractures have been evaluated. 32 of the patients were boy, 16 of them were girl, and the mean age was 4.5 (2-8) years. Under general anesthesia and scopy control the closed reduction achieved by the traction with a K-wire which was passed through femur supracondyllar region, after than immediate hip spica cast wasapplied. Clinical and radiological results were evaluated. Mean follow up time was 30.5 (6-56) months. Mean hospitalization time was 1.7 (1-12) days. Patients were stayed in hip spica cast mean 44 (39-60) days. Results: At last control mean 5 mm of shortness, 2° anterior, 2° varus and 4° valgus angulation determined, but no important complication such as infection and no neurovascular damage was seen. Clinically and radiologically 72 %, 19 % and 9 % of patients had excellent, good and poor results respectively. Conclusions: Because the advantages of the procedure such as short hospitalization period, easy and safe application, less complication rates and no secondary procedures including the removing fixation materials are reasonable, we prefer K- wire incoorperated hip spica cast application for femur diaphyseal fractures in pediatric patients as soon as possible.

Amaç: Çocuk femur cisim kırıklarında acil yapılan kapalı Ayerleştirme ve pelvipedal alçı uygulamasının sonuçlarını değerlendirmektir. Çalışma planı: Kliniğimizde femur cisim kırığı nedeniyle tedavi gören 48 çocuk hasta değerlendirilmiştir (32 erkek, 16 kız, ortalama yaş 4.5 yıl). Acil şartlarda genel anestezi altında ve skopi kontrolünde femur suprakondiller bölgeden geçilen K- telinden traksiyon ve kapalı yerleştirmeyi takiben pelvipedal alçı (PPA) uygulandı. Sonuçlar klinik ve radyolojik olarak değerlendirildi. İzlem süresi ortalama 30.5 (6–56) aydı. Bulgular: Hastanede kalış süresi ortalama 1.7 (1–12) gün, ortalama pelvipedal alçıda (PPA) kalma süresi 44 (39–60) gündü. Son kontrolde femurda ortalama 2 mm kısalık, 2° öne açılanma, 2° varus, 4° valgus açılanması vardı. Damar - sinir yaralanması görülmemiştir. Klinik ve radyolojik olarak çok iyi (% 79), iyi (% 19) ve başarısız (% 9) olarak değerlendirilmiştir. Sonuçlar: Çocuk femur cisim kırıklarında erken kapalı yerleştirme ve PPA uygulamasını; hastanede yatış süresinin kısa, güvenilir ve kolay uygulanır olması, tedavi bitiminde başka girişimlere gerek olmaması nedeniyle önermekteyiz.
