Bisikletler ve travmatik optik nöropati iki çocuk olgunun sunumu

Abstract

Two adolescent boys aged 14 and 15 suffered collisions while riding bicycles with an additional passenger and no protective gear. They presented with unilateral no light perception and Marcus Gunn pupillary reaction. Optic discs of the involved eyes had mild pallor and the margins were slightly obscured. Radiologic imaging revealed no optic canal fractures or intraoptic nerve sheath hemorrhage, however, multiple fractures involving the orbital walls were observed. Treatment was attempted with high dose corticosteroids with no apparent improvement in both patients. Optic neuropathy caused by high velocity injuries sustained with motor vehicles is not uncommon. Although cycling injuries are lower velocity accidents, adolescents can reach high speeds. The addition of a passenger might increase the weight and thereby the momentum of the vehicle especially during down hill riding. These two cases of consecutive optic neuropathies caused by very similar circumstances emphasizes the importance of riding alone and wearing protective head gear during cycling.

Özet

Beraberlerinde ikinci bir yolcu ile beraber ve koruyucu kaskları olmaksızın bisiklete binen 14 ve 15 yaşlarında 2 erkek çocuk bisikletten düşme sonucu tek gözlerinde görme kaybı ile Idiniğimize refere edildiler. Etkilenen taraftaki optik diskler hafif soluk ve minimal ödemli idi. Görüntüleme yöntemleri ile optik kanalda kırık yada optik sinir kılıf içi lıemoraji izlenmezken orbita duvarlarında multipl kırıklar mevcuttu. Her iki hastaya da yüksek doz sistemik steroid tedavisi uygulandı ancak sonuç görme keskinliğinde herhangi bir artış elde edilemedi. Yüksek hızlı motorlu araç kazaları sonrasında travmatik optik nöropati nadir değildir. Bisiklet kazaları yükek hızlarda meydana gelmese de ağırlığı ve dolayısıyla ivmeyi arttıran ek bir yolcu ile yokuş aşağı yollarda çocuklar oldukça yüksek hızlara ulaşabilmektedirler. Benzer koşullarda meydana gelmiş olan bu ild travmatik optik nöropati vakası bisiklete binerken koruyucu kask giymenin önemini vurgulaması açısından önemlidir.
