Abstract

This study covers design, realization and experimental applications of a new phacoemulsification apparatus for cataract surgery. The aim of this study was to design and test a competitively priced Turkish made phacoemulsification system with superior specifications when compared to commercially available ones. The new phacoemulsification system consists of a main unit controlling the whole system and providing commu¬nication via a 7.7" colored touch screen operator panel, a phaco unit, irrigation and aspiration (I/A) units, vitrectomy and diathermy units, custom designed footswitch pedal. This system was designed to function adoptively with respect to hardness of cataracts so that minimum phacoemulsification energy is used, because excessive phacoemulsification energy (ultrasound) may cause irreversible damage to cornea's endothelial cells and to the sensitive inner organs of the eye cambium. The amount of heat produced in the eye by ultrasound depends on the way the ultrasound energy is delivered, in other words, on different modes of phacoemulsification energy. Therefore, experimental research was carried otit to improve a prototype system. Unique modes (UPM-User Pulse Modulation) were designed to minimize phacoemulsification energy. Other unique properties of this system include multifunctional foot-switch pedal, user-friendly pre-operation test menu, aspiration totalizer, and 3 different choices for vitrectomy probe. In conclusion, a unique and Turkish made phacoemulsification system was developed for ophthalmic surgeons for clinical use and research.

Özet

Günümüzde, modern katarakt cerrahisi fakoemülsüflkasyon cihazları ile gerçekleştirilmektedir. Bu çalışma ile ilk Türk malı ve benzerlerinden üstün özellikleri olan ama onlardan daha ucuz bir fakoemülsifikasyon sisteminin tasarlanması, gerçeklenmesi ve kataraktın sertliğine göre minimum fakoemulsifikasyon(ultrason) enerjisi kullanılmasına yönelik çalışmalar yapılması hedeflenmiştir. Bunun nedeni gereğinden fazla uygulanan fakoemülsifikasyon enerjisinin, özellikle korneanın endotel hücreleri başta olmak üzere duyarlı tüm göz içi dokularına geri dönüşümsüz zarar verebilmesidir. Uygulanan fako enerjisinin oluşturduğu ısı miktarlarının çeşitli modlara göre farklı olmasından hareketle, özgün deneysel araştırmalar yapılmış ve prototip sistem geliştirilerek kataraktın sertliğine göre minimum fakoemülsifikasyon enerjisi kullanılmasını sağlayabilen özgün modlar (UPM-User Pulse Modulation) geliştirilmiştir.
