Violence towards Physicians in Turkey: A Grounded Theory Analysis

Abstract
The main purpose of this study is to examine the attitudes and behaviors of physicians in Turkey who have been intensely subjected to violence in recent years and to discover how they bring about the causes of violence. In order to achieve this, it was aimed to deepen the different approaches of violence in social sciences and to reveal possible sociological reasons of violence in the field of health. The main problem of the research is the possibility that the services offered by the physicians in hospitals cannot be fulfilled with full fear of suffering from violence and this situation may be experienced in increasingly wider areas. In the study, in-depth interviews are conducted with 18 physicians working in public hospitals and private hospitals in Ankara and Düzce. Interviews were analyzed based on the principles of the grounded theory. The findings of the research are based on the comparison of quality of service, time allocated for patients, exposing to violence and satisfaction levels. The dissatisfaction from health services of patients and tension of physicians affect the quality of service offered. As a result of this research it has been demonstrated that physicians wanted to get through the process of encountering the patient with the least damage without suffering from violence. As a result of the study, it has been determined that the focal point of violence experienced in patient-physician relation in Turkey is the negative outcomes of health transition program. In addition, within the scope of this research, some health policies have been critically evaluated in Turkey and some suggestions have been made.
Özet
Bu çalışmanın temel amacı, Türkiye’de son yıllarda yoğun olarak şiddete uğrayan hekimlerin şiddete yöneliktutum ve davranışlarını; şiddetin nedenlerini nasıl ortaya koyduklarını keşfetmektir. Bu amaca ulaşabilmekiçin sosyal bilimler literatüründen farklı şiddet yaklaşımlarını sağlık alanında derinleştirilmesi ve sağlıkalanında yaşanan şiddetin sosyolojik olası nedenlerinin ortaya konulması hedeflenmiştir. Araştırmanıntemel problemi, hekimler tarafından hastanelerde sunulan hizmetlerin şiddete uğrama korkusu ile tamanlamıyla yerine getirilememesi durumu ve bu durumun gittikçe genişleyen alanlarda yaşanması ihtimalidir.Çalışmada Ankara ve Düzce kentlerindeki kamu hastaneleri ve özel hastanelerde görev yapan 18 hekim ilegörüşülmüştür. Görüşme metinleri temellendirilmiş kuramın ilkeleri esas alınarak analize tabi tutulmuştur.Hizmet kalitesi, hekimin hastalara ayırmak zorunda kaldığı zaman, şiddete uğrama ve memnuniyetfarklılığı esasında karşılaştırmalara dayanan araştırma bulguları, hekimlerin memnuniyetsizlik, gerilimve gerginliklerinin sunulan hizmet kalitesini etkilediğini; hekimlerin hasta ile karşılaşma sürecini şiddeteuğramadan en az zararla atlatma ya da tamamlamak esasında yürütüldüğünü ortaya koymuştur. Çalışmanınsonucunda, Türkiye’de sağlık alanında hasta-hekim ilişkisinde yaşanan şiddetin odak noktasının sağlıktadönüşümün getirdiği olumsuzlukların olduğu tespit edilmiştir. Ayrıca yine bu araştırma kapsamındaTürkiye’de sağlık politikalarının eleştirel değerlendirilmesi yapılarak bazı önerilerde bulunulmuştur.
