Abstract

The comparison of two different microsurgical techniques used in penil revascularisation. Penil revascularization is a method that must be tried in trauma related occlusion of the penile arterial system which is the most important component of erectile dysfunction or vascular malformations due to congenital anomalies withouth coexistent systemic disease. Currently the most physiologic type of anastomosis in the presence of low pressure in the recipient vessel is the one suggested by Goldstein (modified Michal II) type. The comparison between end to end (Goldstein) and fish mouth (Lue) style anastomosis used in revascularization was aimed in our study. In our study, two seperate groups consisted of ten Albino rats, each. The anastomosis was end to end (Goldstein) type in group 1 and fish mouth type in group 2. At the end to end of our study, patency rates of 80 % and 100 % were determined in end to end and fish mouth type anastomosis, respectively. It was concluded that, the high patency observed in fish mouth type anastomosis is due to the archievement of wide anastomosis field and this would prevent possible anastomosis strictures in later periods.

Özet

Penil revaskülarizasyon; erektil disfonksiyonunun en önemli organik komponentini oluşturan penil arteriyel sistemde travmaya bağlı olan oklüzyonlar veya sistemik bir hastalık olmadan ortaya çıkan kongenital anomalilere bağlı olan damar malformasyonlannda başvurulması gereken bir yöntemdir. Günümüzde alıcı damarda düşük basınç varlığında, en fizyolojik olan anastomoz tipi Goldstein (modifiye Michal II) tarafından önerilendir. Çalışmamızda Goldstein tipi revaskülarizasyonda kullanılan end to end ve balık ağzı (Lue) tarzı anastomoz tiplerinin karşılaştırılması amaçlanmıştır. Her biri 10'ar adet Wistar Albino tipi sıçandan oluşan iki ayrı grup oluşturulmuş, bunlardan grup l'e end to end (Goldstein) tipi, grup 2'ye ise balık ağzı tipi anastomoz uygulanmıştır. Çalışmanın sonunda end to end tarzda yapılan anastomozlarda % 80, balık ağzı tarzda yapılan anastomozlarda ise % 100 oranında geçirgenlik saptanmıştır. Geniş anastomoz yüzeyinin sağlanması nedeni ile balık ağzı tarzındaki anastomozlarda kısa dönemde sağlanan yüksek geçirgenliğin, geniş anastomoz yüzeyine bağlı olduğu ve bunun geç dönemde oluşabilecek muhtemel anastomoz darlıklarını önleyebileceği kanaatine varılmıştır.
