An unusual case of idiopathic hemochromatosis refractory to treatment because of thalassemia trait

Abstract
Idiopathic hemochromatosis is a rare, otosomal recessive genetic disease, whose gene is located near the HLA locus. Diagnosed and treated with venesection early in the course, the prognosis is excellent. We report a case of hemochromatosis presenting with heart failure and diagnosed with transvenous endomyocardial biopsy. The concurrent presence of beta thalassemia minor and splenectomy prevented the effective use of venesection and accelerated the course of the disease and resulted in early demise.
Özet
İdyopatik hemokromatozis nadir görülen HLA lokusuna yakın gen ile ilişkili otosomal resesif geçen kalıtımsal bir hastalıktır. Erken tanı konulan ve filebotomi uygulanan hastalarda prognoz oldukça iyidir. Bu yazıda kalp yetersizliği semptomları ile başvuran, transveneöz endomyokardiyal biyopsi ile hemokromatoz tanısı konulmuş bir vaka sunulmuştur. Hastada beta talassemi minör varlığı ve daha önce splenektomi yapılmış olması, flebotominin etkin olarak uygulanamamasına neden olup hastalığın seyrini hızlandırmış ve vakanın erken dönemde kaybedilmesine neden olmuştur.
