Abstract

In this study, patients who had been operated for primary chest wall tumors in our department between 1987 and 1997 were evaluated retrospectively. 39 patients, 25 male and 14 female, between 14 and 68 years of age were undergone for 41 chest wall resections. When evaluated histopathologically; 26 cases were found out to be malignant and 15 to be benign. Parsial resections of 1-6 costas for 30 patients, parsial resections of sternum, clavicle and 1st costa for 5 cases and soft tissue resections for 6 cases were performed. Reconstructive surgery was needed for 23 cases. Polytetrafluoroethilene grefts, muscle flaps and combinations of methyl methacrylate and synthetic grafts were used for reconstructions. The effects of histopathological diagnose, surgical resection and reconstruction and postoperative adjuvant therapy on survival were evaluated in our study.

Özet

Bu çalışmada, Anabilim Dalımızda 1987-1997 yılları arasında primer toraks duvarı tümörü sebebiyle opere edilen hastalar retrospektif olarak değerlendirildi. Yaşları 14-68 arasında değişen 25' i erkek, 14' ü kadın toplam 39 hastaya 41 toraks duvarı rezeksiyonu uygulandı. Olgular histopatolojik olarak incelendiğinde; 26'sının malign, 15' inin benign karakterde olduğu tespit edildi. Cerrahi girişim olarak; 30 olguda 1-6 kosta parsiyel rezeksiyonu, 5 olguda kosta, klaviküla ve sternum parsiyel rezeksiyonu, 6 olguda yumuşak doku rezeksiyonu uygulandı. 23 olguya rekonstrüktif girişim uygulamak gerekti. Yapılan rekonstrüksiyonlarda; politetrafloroetilen greft, kas flepleri, metil metakrilat ve sentetik greft kombinasyonları uygulandı. Çalışmamızda, primer toraks duvarı tümörlerinde histopatolojik tanının, uygulanan cerrahi girişimin ve postoperatif adjuvan terapi uygulamasının sürvi üzerine etkisi ve uyguladığımız cerrahi rezeksiyon ve rekonstrüksiyon teknikleri tartışılmaktadır.
