Abstract

Antiphospholipid syndrome (APLS) is characterized by tendency to thrombosis, recurrent fetal losses and thrombocytopenia in the presence of antiphospholipid antibodies (Lupus anticoagulants (LA) and anticardiolipin antibodies). Because LA were found to be strongly associated with thromboembolic complications in recent prospective studies, the importance of laboratory diagnosis of LA increased. In this study 117 patients with APLS were tested for LA with activated partial thromboplastin time (aPTT) kaolin clotting time (KCT), mixing test and platelet neutralization test. Also, patient and normal plasma were filtered with 0.22µ plazma filter and were tested again. Before the plasma filtration, LA were found to be positive only in 53.8% of patients when aPTT was used, and in 77.7% of patients when KCT was used. Antifosfolipid sendromu, antifosfolipid antikorlarının (Lupus antikoagülanı (LA) ve antikardiolipin antikorları) varlığında tromboza eğilim, tekrarlayan düşükler ve trombositopeni ile karakterizedir. Son yıllarda yapılan prospektif çalışmalarda LA ile trombotik komplikasyonlar arasında güçlü bir ilişki olduğunun gösterilmesi, LA' nın laboratuar teşhisinin önemini arttırmıştır. Bu çalışmada antifosfolipid sendromlu 117 hastada, aktive parsiyel tromboplastin zamanı (aPTZ) ve kaolin pıhtılaşma zamanı (KPZ) yöntemleri ile, karışım testi ve trombosit nötralizasyon testi kullanılarak LA aranmıştır. Ayrıca hasta plazmaları ve normal plazmalar 0.22 µ'luk plazma filtreleri ile filtre edilip tekrar test edilmişlerdir. Plazma filtrasyonu öncesinde aPTZ ile hastaların sadece %53.8'inde, KPZ ile %77.7'sinde LA pozitif bulunabilmiştir. Filtrasyon işlemi yapıldığında bu oranlar aPTZ için %93.1' e, KPZ için %100' e ulaşmıştır. Plazma filtrasyonunun her iki testin de duyarlılığını istatistiksel olarak anlamlı bir şekilde arttırdığı görülmüştür (x2, aPTZ için p
