Serum interleukin-6 levels in patients with non-hodgkin's lymphoma

Abstract
Interleukin-6 (IL-6) is a multipotent lymphokin that plays a central role in normal B-cell maturation and in proliferation of some B-cell malignancies including multiple myeloma. In this study we investigated serum IL-6 concentration in untreated patients with non-Hodgkin's tymphoma (NHL) and its relation to the clinical outcome. Serum levels of IL-6 were measured by using enzyme linked immunosorbent assay. The study group was consisted of 21 patients (age between 18 and 86 year) and control group was consisted of 19 healthy adult (age between 26 and 64 year). According to R.E.A.L. classification, 15 of 21 patients had aggresive lymphoma and 6 of 21 patients had indolant lymphoma. Six patients with aggresive lymphoma had died during 6-24 months follow-up period. Fourteen patients had systemic B symptoms. in all patients of serum IL-6 levels were higher than controls but the difference was not statistically significant (23.00 ± 22.11 pg/ml, 14.15 ± 3.93 pg/ml, p>0.05). Serum IL-6 levels of the patients with aggresive lymphoma were higher than low-grade lymphomas (28.94 ± 23.32 pg/ml, 9.15 ± 10.7 pg/ml, p=0.04). Patients with B symptoms had higher serum IL-6. levels than patients without B symptoms (32.13 ± 22.07 pg/ml, 6.05 ± 7.63 pg/ml, p=0.0008). Patients who died soon after been diagnosed as NHL had higher serum levels of IL-6 than patients still alive (41.50 ± 21.25 pg/ml, 16.84 (19.27 pg/ml, p=0.003). As a results elevated serum IL-6 in NHL patients may be an indicator of more aggressive course.

İnterlökin-6 (IL-6) multipotent bir lenfokin olup, multipl myeloma başta olmak üzere bazı malign hastalıklarda yükselebilmektedir. Bu çalışmada tedavi görmemiş Hodgkin dışı lenfomalı (HDL) olgularda serum IL-6 düzeylerinin, hastaların klinikopatolojik özellikleri ile ilişkisi araştırılmıştır. Serum IL-6 düzeyleri ELISA yöntemi ile tayin edilmiştir. Çalışma grubu yaşları 18-86 arasında HDL'li 21 hastadan, kontrol grubu ise yaşları 26-64 arasında 19 sağlıklı erişkinden oluşmuştur. Patolojik olarak R.E.A.L. sınıflamasına göre 15 hastada agresif, 6 hastada yavaş seyirli lenfoma mevcuttu. Sistemik semptomların varlığına göre 14 hasta B, 7 hasta A evresinde idi. Altı ile 24 aylık takiple, agresif lenfomalı hastaların 6'sı vefat etmiştir. Hastaların serum IL-6 düzeyleri kontrol grubunda yüksek bulunmakla birlikte aralarındaki fark anlamlı değildi (sırası ile, 23.00 ± 22.11 pg/ml, 14.15 ± 3.93 pg/ml, p>0.05). Agresif lenfomalı hastaların serum IL-6 düzeyleri, yavaş seyirli lenfomalı hastalardan daha yüksek idi. (sırası ile, 28.94 ± 23.32 pg/ml, 9.15 ± 10.7pg/ml, p= 0.04). B evresindeki hastaların serum IL-6 düzeyleri de evre A'daki hastalardan yüksek idi (sırası ile, 32.13 ± 22.07 pg/ml, 6.05 ± 7.63 pg/ml, p=0.0008). Takiplerinde vefat eden hastaların tanı konulduğu sıradaki serum IL-6 düzeyleri, yaşayanlardan daha yüksekti (sırası ile, 41.50 ± 21.25 pg/ml, 16.84 ± 19.27 pg/ml, p=0.003). Sonuç olarak, serum IL-6 düzeylerinin yüksek bulunması, HDL'nin daha agresif bir seyir izleyeceğinin göstergesi gibi görünmektedir.
