Abstract

Bickerstaff Brainstem Encephalitis: A Case Report. Bickerstaff s brainstem encephalitis (BBE) is a monophasic, post-infectious disorder with acute ophtalmoplegia, ataxia, drowsiness, extensor plantar responses and hemi sensory loss. The etiology of BBE is unknown, but an autoimmune mechanism has been proposed. There are limited number of studies indicating the benefits of plasmapheresis, corticosteroids and IV immunoglobulin in treatment. The case was almost completely cured with IV immunoglobulin treatment. The clinical progress, elect-rophysiological findings and treatment results of a case of BBE with central and peripheral involvement was presented and discussed due to its rare incidence.

Özet

Bickerstaff beyin sapı ensefaliti (BBE); oftalmopleji, ataksi, bilinç bozukluğu, extensor plantar yanıt ve hemihipoestezinin bulunduğu, santral sinir sistemi tutulumu ile birlikte periferik tutulum da gösterebilen, monofazik post-infeksiyoz bir klinik tablodur. BBE'nin etiolojisi tam olarak bilinmemekle beraber otoimmun bir mekanizma sonucu geliştiği ileri sürülmektedir. Tedavide plazmaferez, kortikosteroid ve intravenöz immunglobulin (IVIg) kullanımının yararlı olduğunu bildiren, az sayıda çalışma mevcuttur. Santral ve periferik sinir sistemi tutulumu ile ' seyreden ve intravenöz immunglobulin tedavisi ile tama yakın iyileşme gösteren bir BBE olgusu, literatürde az rastlanması nedeniyle, klinik seyir, elektrofizyolojik bulgular ve tedavi sonuçları açısından tartışılmaya değer bulundu.
