An unusual complication of acute appendicitis: Perinephritic abscess

Abstract

Acute appendicitis may occasionally become extraordinari- ly complicated and create difficulties in diagnosis, and may be life-threatening. Herein, is described a 50 year-old man who was hospitalized in the gastroenterology clinic due to fatigue, nausea, and fever for the last 10 days. Computed tomography of abdomen revealed abscess in the right retro- peritoneal space involving perinephritic region. Laparotomy and retroperitoneal exploration were performed immediate- ly and pus was drained. A ruptured retrocecal appendix was confirmed as the cause of abscess. Perforated acute appen- dicitis can occasionally manifest as complicated retroperi- toneal abscess without remarkable abdominal symptoms. Thus, it is necessary to maintain a high index of suspicion in a patient with symptoms of retroperitoneal infection.

Akut apandisit zaman zaman sıra dışı komplike hale gelebilir ve tanıda zorluğa neden olarak hayati tehlike oluşturabilir. Bu yazıda, gastroenteroloji kliniğine son 10 gündür halsizlik, bulantı ve ateş şikayeti ile yatırılan 50 yaşında hasta sunuldu. Abdominal tomografi perinefritik bölgeyi tutan sağ retroperitoneal apseyi gösterdi. Acil laparotomi ve retroperitoneal eksplorasyon ile pü drenajı yapıldı. Apsenin nedeni olarak perfore apandisit saptandı. Perfore apandisit bazen karın ağrısına neden olmadan perinefritik apse ile ortaya çıkabilir. Bu nedenle retroperitoneal enfeksiyonu düşündüren semptomları olan hastalarda perfore apandisitten yüksek oranda şüphelenmekgereklidir.
