Papillary thyroid carcinoma emerging with choroid metastasis

Abstract

Patient, seventy five years old, female, applied with decreased vision at her left eye for 4 months. Vision of the patient was as such; 0.2 for right and temporally counts fingers from 1 meter for left eye. A mass at 6 o’clock position of the left eye, which was lying down to vitreous, was detected on fundus examination. The patient was accepted to the service with the diagnosis of malign melanoma and left enucleation operation was performed under general anesthesia. Pathological inspection of enucleation material was resulted as papillary thyriod carcinoma metastasis to choroid and retina. Differential diagnosis of primary and metastatic tumour of the eye must be done carefully.

Özet

Yetmiş beş yaşında, kadın hasta, 4 ay önce başlayan sol gözde görme azlığı yakınması ile başvurdu. Başvuru esnasında görme keskinliği sağda 0,2, solda temporalden 1 metreden parmak sayma düzeyinde idi. Yapılan fundus muayenesinde sol gözde, saat 6 hizasında vitreye uzanan kitle saptandı. Hasta sol malign melanom ön tanısı ile servise kabul edildi ve genel anestezi altında sol enükleasyon ameliyatı uygulandı. Enükleasyon materyali patolojik incelemeye gönderildi. Patolojik inceleme sonucu koroid ve retina invazyonu gösteren tiroid karsinomu (papiller tip) metastazı olarak geldi. Metastatik ve primer göz içi tümörleri ayırıcı tanısı dikkatli yapılmalıdır.
