Protocol for Nursing Care in Endoscopic Retrograde Cholangiopancreatography in the Endoscopy Unit

Abstract
Objective: Endoscopic Retrograde Cholangiopancreatography (ERCP) is an invasive method used in the diagnosis and therapy of pancreaticobiliary diseases. Pre- and post-procedural preparation, follow up and manage- ment of ERCP patients by the associated nursing team play a critical role. Serious complications may occur, especially after invasive therapeutic pro- cedures in ERCP. In such patients, nursing care in patient management is a vital part of the procedure. Methods: Between 2009 and 2011, a predefined protocol was applied to the treatment of ERCP patients in our unit. This protocol included pre- procedural 8-hour fasting, a prophylactic single-dose first-line cephalos- porin injection, postprocedural close nursing care and a serum biochemi- cal panel performed at 3-24 hours. Pre-procedural and post-procedural diagnosis, procedure details, complications and discharge time were all recorded in our database. Results: During the study period, we performed 140 ERCP procedures. Mec- hanic biliary obstruction and extraction was the most common indication and therapeutic procedure, respectively. Hyperamylasemia was found in 45 patients after the procedure. Perforation, which is treated surgically, mic- roperforation and pancreatic pseudocyst were found to have developed in one patient each. 85% of patients were discharged the morning after the procedure. Conclusion: In the treatment of hepatopancreatobiliary diseases, ERCP is a commonly used invasive procedure that carries a high risk of mortality. Postprocedural close care and management is essential to decrease the risk of mortality. Sufficient technical grounding in the use of equipment, as well as the education and experience of ERCP nurses, are also important factors in avoiding the complications that can occur after this challenging ERCP procedure

Amaç: Endoskopik Retrograd Kolanjio Pankreotografi (ERCP) safra yolları ve pankreas hastalıklarının tanı ve tedavisinde kullanılan, invaziv bir giri- şimdir. ERCP uygulanan hastalara işlem öncesi ve sonrasında uygulanacak hazırlık, takip ve tedavide hemşirelik bakısı oldukça büyük önem taşımaktadır. Özellikle tedavi edici ERCP sonrası çeşitli komplikasyonlar gelişebilmektedir. Komplikasyon gelişmiş ya da olası komplikasyon beklentisi olan hastalarda hemşirenin yakın takibi gereklidir. Yöntemler: Kliniğimizde 2009-2011 yılları arasında ERCP yapılan hastalara aynı protokol uygulanmaktadır. Hastalara 8 saatlik açlığı takiben ERCP yapılmakta, işlem öncesi tek doz 1. kuşak sefalosporin yapılmakta, işlem sonrası 2 saat aralıklarla takibi yapılmakta, 3. ve 24. saatlerde protokoldeki tetkikleri alınmaktadır. Çalışmada ERCP öncesi tanılar, uygulanan girişimsel işlemler, kesin tanılar, işlem sonrası gelişen komplikasyonlar ve hastaların taburcu saatleri kaydedildi. Bulgular: Belirtilen dönemde toplam 140 hastaya ERCP işlemi uygulanmıştır. En sık tıkanma ikteri tanısıyla işlem yapılmıştır. Koledoktan taş ekstraksiyonu en sık uygulanan girişimdi. Hastaların 45'inde erken dönemde hiperamilazemi gelişmiştir. Bir hastada cerrahi tedavi gerektiren perforasyon, 1 hastada mikroperforasyon ve 1 hastada pankreas psödokist gelişimi saptanmıştır. Hastaların %85'i ertesi sabah taburcu edilmiştir. Sonuç: ERCP hepato pankreato biliyer cerrahi hastalarında sık uygulanan, birçok komplikasyonlarla ve nadir olarak görülse de mortalite ile sonuç- lanabilen invaziv bir girişimdir. Titiz bir hazırlık ve işlem sonrası hastaların yakın takibi gereklidir. Bu nedenle yeterli teknik alt yapı, alet ve sarf malzemelerine ek olarak, hemşirenin de bu işlem hakkında bilgisi ve deneyiminin olmalıdır.
