A Rare Type of Presentation of Lymphoma: Primary Cerebral Lymphoma

Abstract

Primary central nervous system (CNS) lymphoma is a rare form of extran- odal non-Hodgkin’s lymphoma. It constitutes 1% of all intracranial tu- mours. The most common histologic subtype is high grade diffuse large B cell lymphoma. It commonly presents with neurological signs and ocular symptoms. The diagnosis requires histopathologic confirmation of lym- phoma and exclusion of the lymphoma involvement at sites other than the CNS. Its treatment usually includes a combination of chemotherapy and radiotherapy. Here, we report a patient who was diagnosed with pri- mary CNS lymphoma at our clinic. The patient presented with neurologic and ocular symptoms. A cerebral mass was identified on MRI. A histo- logical diagnosis of lymphoma was made after surgical excision of the cerebral mass. No lymphoma involvement was found on PET scan and bone marrow biopsy. Thus, the patient was diagnosed with primary ex- tranodal CNS lymphoma

Primer merkezi sinir sistemi (MSS) lenfomaları ekstranodal non-Hodgkin lenfomanın nadir görülen bir şeklidir. Tüm intrakraniyal tümörlerin %1'ini oluşturur. En sık görülen histolojik alt tipi yüksek dereceli diffüz büyük B hücreli lenfomadır. En sık nörolojik ve oküler belirtilerle kendini gösterir. Tanı histopatolojik olarak lenfomanın saptanması ve merkezi sinir sistemi dışındaki odaklarda lenfoma tutulumunun dışlanmasıyla konur. Tedavi genellikle kemoterapi ve radyoterapi bileşimini içerir. Burada; kliniğimizde primer MSS lenfoması tanısı alan bir olgu ele alındı. Hasta oküler ve nörolojik belirtilerle başvurdu. MRI incelemesinde bir kraniyal kütle lezyon tespit edildi. Kraniyal kütlenin cerrahi olarak çıkarılmasından sonra lenfoma histopatolojik tanısı kondu. PET taraması ve kemik iliği biyopsisinde lenfoma tutulumu görülmedi. Böylece; hasta primer ekstranodal MSS lenfoması tanısı aldı.
