Surgical Splenectomy Indications in Benign and Malignant Splenic Diseases and the Role of Minimally Invasive Surgery

Abstract

Objective: Surgical splenectomy is one of the most frequent operations car- ried out by surgeons specializing in trauma and hematologic diseases. The techniques for splenectomy are open surgical splenectomy, hand-assisted laparoscopic splenectomy (HALS), laparoscopic splenectomy (LS) and single- incision laparoscopic splenectomy (SILS). In this study we aim to discuss our experiences and evaluate the optimum surgical technique for splenectomy. Methods: A retrospective analysis of 74 patients who had undergone sur- gical splenectomy was carried out. 65 patients who had undergone open splenectomy and 9 patients who had undergone laparoscopic splenectomy were compared with regard to operation indications, splenic size, age, sexu- ality, operation time, peroperative blood loss, postoperative complications and length of stay in hospital. Results: Of the 74 patients who had undergone splenectomy, 50 had hema- tologic diseases and 9 had hydatid cysts. Of the patients who had undergone open splenectomy, the splenic size was no larger than 57 cm. In those pati- ents that had undergone laparoscopic splenectomy, the splenic size was no larger than 21 cm. The average age of open splenectomy patients was 42.6 years (with the youngest being 9 years old and the eldest 83). The average age of laparoscopic surgery patients was 41.2 years (with the youngest being 17 and the eldest 54). The female: male ratio in open splenectomy patients was 32:33, and in laparoscopic splenectomy patients was 5:4. Of the para- meters comparing preoperative, peroperative, and postoperative bleeding, the peroperative bleeding parameters were found to be statistically signi- ficant. Peroperative blood loss in open splenectomy was found to be too great. Other parameters were not found to be statistically significant. Conclusion: The past experience of surgeons and the splenic size should direct our approach to surgical splenectomy. In the case of hematologic diseases and malignant splenic masses, open surgical splenectomy is a ne- cessary approach

Amaç: Başta travma ve hematolojik hastalıklar olmak üzere cerrahi splenektomi genel cerrahların sıkça yaptığı ameliyatlardandır. Açık cerrahi splenektomi (AS), el yardımlı laparoskopik splenektomi (HALS), laparoskopik splenektomi (LS) ve tek kesiden laparoskopik splenektomi (SİLS); splenektomi için günümüzde kullanılan cerrahi tekniklerdir. Biz yazımızda kendi kliniğimizin deneyimini ve splenektomi için en avantajlı cerrahi tekniği tartışmayı amaçladık. Yöntemler: Cerrahi splenektomi uygulanan toplam 74 hasta dosyası retrospektif olarak incelendi. Açık cerrahi uygulanan 65 hasta ile laparoskopik splenektomi uygulanan 9 hasta ameliyat endikasyonları, dalağın boyutu, yaş, cinsiyet, ameliyat süresi, ameliyattaki kan kaybı, postoperatif komplikasyonlar, hastanede kalış süresi bakımından karşılaştırıldı. Bulgular: Splenektomi uygulanan 74 kişinin 50 tanesinde hematolojik hastalık vardı, ikinci sıklıkta kist hidatik 9 vaka ile yer alıyordu. AS uygulanan hastalarda dalak boyutu 57 cm'ye kadar ulaşırken, LS'de 21 cm en büyük boyuttu. Yaş ortalaması AS'de 42,6 yıl (9-83), LS'de 41,2 yıl (17-54), cinsiyet (K:E) oranı AS'de 32:33, LS'de 5:4 olarak bulundu. Karşılaştırılan preoperatif, peroperatif ve postoperatif parametrelerden istatiksel olarak anlamlı bulunan (p=0,04) peroperatif kanama oldu. AS'de bu değer fazlaydı. Diğer parametrelerde ise anlamlı fark yoktu. Sonuç: Cerrahın deneyimi ve masif dalak büyüklüğü splenektominin hangi yolla yapılması gerektiğini yönlendirmektedir. Özellikle hematolojik hastalıklarda ve malign dalak kitlelerinde cerrahi splenektomi gerekli bir yaklaşımdır
