Bilateral Anterior Cerebral Artery Territory Infarction Due to Subarachnoid Hemorrhage: Two Case Reports

Abstract

Bilateral infarction in the anterior cerebral artery vascular territory is very rarely seen and usually results from rupture of an aneurysm of the ante- rior communicating artery or from thrombosis of the precommunicating part of the anterior cerebral artery in combination with an agenesis of the contralateral part. Our first case, a 40 year old right-handed man, pre- sented with headache and tomography of the brain showed a subarach- noid hemorrhage. In the follow-up process, magnetic resonance imaging of the brain was made upon development of paraparesis, ischemic infarct was detected in the bilateral anterior cerebral artery territory and it was considered that the reason of infarction could be vasospasm. In his digital subtraction angiography, multiple aneurysms were detected. Our second case, a 57 year old right-handed man, presented with a sudden loss of consciousness, tomography of the brain showed subarachnoid hemor- rhage. Embolization was applied upon detecting an aneurysm of the ante- rior communicating artery in his digital subtraction angiography,and con- fusion, total aphasia, tetraparesis and urinary incontinence occurred after embolization. Magnetic resonance imaging of the brain showed acute ischemic infarction in the bilateral anterior and median cerebral artery territory. Embolism and vasospasm were thought to be the reason for the infarction. Because of being rare, our cases were presented together with the relevant literature

Özet

Bilateral anterior serebral arter enfarktları nadir görülür ve genellikle anterior komünikan arter anevrizma rüptürü sonucu veya anterior serebral arterin kontralateral agenezisinin eşlik ettiği prekomünikan alandan geli- şen tromboz sonucu oluşur. Birinci olgumuz 40 yaşında sağ el dominant erkek hasta, kliniğimize baş ağrısı ile başvurdu, bilgisayarlı beyin tomografisinde subaraknoid kanama saptanması üzerine servisimize interne edildi. Takip sürecinde hastada paraparezi gelişmesi üzerine yapılan beyin manyetik rezonans görüntülemesinde bilateral anterior serebral arter alanında iskemik enfarkt saptandı ve gelişen enfarktın vazospazm nedenli olabileceği düşünüldü. Hastanın yapılan dijital subtraksiyon anjiografisinde multiple anevrizmalar saptandı. İkinci olgumuz 57 yaşında sağ el dominant erkek hasta, kliniğimize ani gelişen bilinç kaybı ile başvurdu, bilgisayarlı beyin tomografisinde subaraknoid kanama saptandı. Dijital subtraksiyon anjiografisinde anterior kominikan arter anevrizması saptanması üzerine embolizasyon uygulanan hastada işlem sonrasında konfüzyon, total afazi, tetraparezi, idrar inkontinansı gelişti. Beyin manyetik rezonans görüntülemesinde bilateral anterior serebral arter ve median serebral arter alanlarında akut iskemik enfarkt saptandı. Enfarkta vazospazmın neden olduğu düşünüldü. Olgularımız nadir görülmeleri nedeniyle literatür bilgileri eşliğinde sunulmuştur
