The Effects of Spinal Anesthesia on the Results of Percutaneous Nephrolithotripsy

Abstract

Objective: The effects of spinal anaesthesia on the results of percutaneousnephrolithotripsy (PNL) was evaluated.Methods: From June 2010 to March 2011, 42 patients [Group 1 (spinalanaesthesia, n=21), Group 2 (general anaesthesia, n=21)], treated withstandard percutaneous nephrolithotripsy due to renal stones, were enrolledin this study. The groups were compared in terms of operation duration,hospital stay, analgesic requirement, hemoglobin drop, success rate,complications and auxilliary procedures.Results: There was no difference between the two groups in terms of operationduration, hospital stay, analgesic requirement, hemoglobin drop,success rate, complications and auxilliary procedures. However, postduralpuncture headache was observed in 3 (14.2%) patients, and 2 of themwere re-hospitalized due to severe headache. The patients were treatedwith an epidural blood patch.Conclusion: Spinal anesthesia may not cause negative effects on the resultsof standard PNL, and it may be a good alternative to general anesthesiain selected patients. However, it can cause severe postdural punctureheadache which require re-admisson for treatment in some patients

Amaç: Spinal anestezinin standart perkütan nefrolitotripsi (PNL) sonuçlarınaolan etkisi değerlendirildi.Yöntemler: Bu çalışmaya Haziran 2010-Mart 2011 tarihleri arasında böbrektaşı nedeniyle PNL uygulanan 42 olgu [Grup 1 (spinal anestezi, n=21),Grup 2 (genel anestezi, n=21)] dahil edildi. Gruplar operasyon süresi, hastanedekalış süresi, analjezik gereksinimi, hemoglobin düşüşü, başarı oranı,komplikasyonlar ve ek girişimler açısından karşılaştırıldı.Bulgular: İki grup arasında operasyon süresi, hastanede kalış süresi, analjezikgereksinimi, hemoglobin düşüşü, başarı oranı, komplikasyonlar veek girişimler açısından anlamlı farklar saptanmadı. Bununla birlikte, 3(%14.2) olguda spinal anestezi ile ilişkili baş ağrısı gözlendi. Bu olgularınikisi şiddetli baş ağrısı nedeniyle kliniğe yatırıldı ve epidural kan yamasıile tedavi edildi.Sonuç: Spinal anestezi standart PNL sonuçlarını olumsuz etkilemeyebilirve seçilmiş olgularda genel anestezi uygulamasına iyi bir alternatif olabilir.Bununla birlikte, bazı olgularda tedavi için kliniğe yatırmayı gerektirecekşiddetli baş ağrılarına yol açabilir
