Spinal Tumors: Retrospective Analysis in 45 Cases

Abstract
Objective: Spinal tumors are classified as extradural, intradural-extra-medullary, and intradural-intramedullary according to their localization. The most commonly seen extradural spinal tumors are metastatic tumors. Meningiomas and neurinomas are mostly seen in the intradural-extra-medullary group and ependymomas and astrocytomas in the intradural intramedullary group.Methods: In this clinical study, we analyzed 45 spinal tumor cases op-erated between January 2009-August 2013 retrospectively and presented their results with the clinical evaluation.Results: Twenty-two of the patients were male, and 23 were female. Mean age was 53.8 (range 18-80). According to localization, 24 cases were ex-tradural, 17 cases were intradural-extramedullary, and 4 cases were in-tramedullary. Five of the cases were located in the cervical, 27 were in the thoracal, and 13 were in the lumbar region. Total excision was done in all the patients. Instrumentation was not applied, since instability was not observed.Conclusion: Spinal tumor causes radicular or local pain and neurologic deficits. Immediate surgical intervention is necessary as soon as the neu-rologic deficit develops. Total resection must be done; however, subtotal resection can be done in infiltrative intramedullary tumors in order not to let additional neurological deficits occur. Prognosis is related to the de-velopment period of preoperative symptoms, the degree of deficits, and histopathological diagnosis.

Amaç: Spinal tümörler yerleşim yerlerine göre ekstradural, intradural-eks-trameduller, intradural-intrameduller olarak sınıflandırılır. En sık görülen ekstradural tümörler metastazlardır. İntradural-ekstrameduller tümörler arasında meningiomlar ve nörinomlar, intradural intrameduller tümörler olarak da en sık epandimomlar ve astrositomlar görülür.Yöntemler: Bu çalışmada Ocak 2009-Ağustos 2013 seneleri arasında klini-ğimizde ameliyat edilmiş olan 45 spinal tümör olgusu ve ameliyat sonrası sonuçları sunulmuştur. Bulgular: Olgulardan 22'si erkek, 23'ü kadındır. Ortalama yaş 53,8'dir (yaş aralığı 18-80 yaş). Yerleşim yerlerine göre 24 olgu ekstradural, 17 olgu intradural-ekstramedüller, 4 olgu ise intramedüller yerleşimlidir. Bu ol-gulardan 5'i servikal, 27'si torakal ve 13'ü lomber bölgededir. Hastaların hepsine total eksizyon yapılmış, ameliyat sonrası instabilite düşünülmedi-ğinden enstrümentasyon uygulanmamıştır. Sonuç: Spinal tümörler radiküler veya lokal ağrıya ve nörolojik defisite neden olurlar. Nörolojik defisit gelişmeye başladığı anda acil cerrahi mü-dahale gereklidir. Cerrahide total rezeksiyon gerekli olmakla birlikte int-rameduller infiltratif tümörlerde ek nörolojik defisitten kaçınmak için re-zeksiyon subtotal olabilir. Prognoz, ameliyat öncesi semptomların gelişme süresi, defisitlerin derecesi ve histopatolojik tanıyla bağlantılıdır.
