Posterior Approach in Cervical Disc Herniations

Abstract

Objective: The preferred approach in the treatment of cervical disc herniation is decided by the surgeon's clinical experience and remains controversial. Posterior "key hole" approach is a less applied technique compared to the anterior approach. The aim of this clinical study is to evaluate the clinical, radiologic, and post-operative results of the patients who went posterior cervical laminoforaminotomy.Methods: The clinical, radiological, and post-operative results of the 14 patients operated by posterior cervical approach between October 2010 and August 2013 were retrospectively analyzed.Results: Of 14 patients operated who underwent posterior surgery, 12 were male and 2 were female; mean age was 42.8 (range 33-53 years). In all of the patients, the neurologic deficit was observed as one-sided radiculopathy (100%). In 9 of the patients, the disc level was at C6-7, 3 of the patients at C5-6, and 2 of the patients at both C5-6 and C6-7; mag-netic resonance imaging (MRI) showed soft disc herniation in 10 of the 14 patients (71.4%). Nine of the 10 patients accessible went through the complaints after the surgery (90%). Conclusion: Posterior cervical laminotomy-uncusectomy is an appropri-ate choice in cervical disc herniations. Also, good results are taken in pos-terior decompression surgery, even in lateral hard disc herniation cases.

Amaç: Servikal disk hernisine anterior veya posteriordan yaklaşımı genel-likle cerrahın alışkanlığı belirlemektedir. Servikal disk hernilerinde poste-rior "key hole" yaklaşım, anterior girişime göre daha az uygulanmaktadır. Bu çalışmanın amacı posterior servikal laminoforaminotomi yapılmış ser-vikal disk hernili hastaların klinik, radyolojik ve post-operatif sonuçlarını değerlendirmektir. Yöntemler: Ekim-2010 ve Ağustos-2013 seneleri arasında posterior servi-kal yaklaşımla ameliyat edilmiş 14 hastanın, klinik, radyolojik bulguları ve post-operatif sonuçları tartışılmıştır. Bulgular: Posterior yaklaşımla ameliyat edilmiş 14 servikal disk hernili hastanın 12'si erkek, 2'si kadındı, ve ortalama yaş 42,8 (yaş aralığı 33-53 yıl) idi. Hastaların tümünde nörolojik defisit tek taraflı radikülopati olarak gözlendi (%100). Dokuz olguda disk C6-7 seviyesinde, 3 olguda C5-6 sevi-yesinde, 2 olguda C5-6 ve C6-7 seviyesinde idi. Manyetik rezonans görün-tülemede (MRG) yumuşak disk, posterior girişim uygulanan 14 olgunun 10'unda (%71,4) mevcuttu. Şikayetlerin geçmesi ulaşılabilen 10 hastanın 9'undadır (%90). Sonuç: Posterior sevikal laminotomi-uncussektomi birçok servikal disk hernisinde uygun bir seçenektir. Ayrıca posterior dekompresyon lateral sert disk tanılı hastalarda bile iyi sonuç verebilir.
