Dysferlinopathy: A Case Report and Literature Update

Abstract
Dysferlinopathy is a rare autosomal recessive myopathy, resulting in the lack or absence of dysferlin production caused by mutations in the encoding gene. Dysferlin is a sarcolemmal membrane protein in- volved in the repair of membrane damage caused by calcium. There are four identified phenotypic dysferlinopathies, two of which are relatively frequently observed. Miyoshi myopathy and limb-girdle muscular dystrophy type 2B are frequently observed; the two very rare dysferlinopathies are distal anterior compartment myopathy and scapuloperoneal muscular dystrophy (observed in only one case). Se- rum CK levels are quite high, even in the pre-clinical period. A muscle biopsy typically shows dystrophic patterns, often accompanied with T-lymphocyte-based inflammatory changes. The clinical course of dysferlinopathy is usually much better than that of other recessive trait muscular dystrophies. Dysferlinopathies should be considered in the differential diagnosis of polymyositis to avoid unnecessary and potentially dangerous medications such as oral steroids or immuno- suppressive therapies. Here we report the case a 21-year-old Syrian patient diagnosed with dysferlinopathy who has had serious CK eleva- tions from the age of 1 and who had been diagnosed with polymyo- sitis by a muscle biopsy 7 years ago and who therefore used steroids/ azathioprine for the following 3 years

Disferlinopati, disferlin proteinini kodlayan gende mutasyon sebebi ile disferlin proteininin eksikliği veya yokluğu sonucu oluşan, otozomal resesif kalıtılan nadir görülen bir myopatidir. Disferlin, kalsiyum aracılı sarkolemma hasarı tamirinde rol oynayan bir membran proteinidir. Farklı fenotipik görünümlerde başlıca Myoshi miyopati (MM) ve limb-girdle müsküler distrofi (LGMD)-Tip 2B olmak üzere; distal anterior kompartman miyopati ve sadece 1 vakada tanımlanmış skapuloperoneal müsküler distrofi olmak üzere 4 tipi tanımlanmıştır. Klinik görünüm otozomal resesif kalıtılan diğer müsküler distrofilere göre oldukça selimdir. Serum CK düzeyi preklinik dönemde dahi çok ciddi yüksektir. Kas biyopsisinde tipik olarak distrofik patern görülüp sıklıkla T lenfosit ağırlıklı imflamatuvar değişiklikler eşlik eder. Disferlinopatilerin polimiyozitlerin ayırıcı tanısında düşünülmesi, uzun süreli oral steroid veya immun süpresan tedaviler gibi gereksiz ve potansiyel tehlikeli girişimlerden kaçınmak için son derece önemli olacaktır. Burada 1 yaşından itibaren CK yüksekliği sebebi ile takipli, Suriye'de 7 yıl önce yapılan kas biyopsisi sonucu polimiyozit tanısı ile 3 yıl boyunca steroid/azatioprin tedavileri almış hastaya disferlinopati tanısı koyduğumuzu bildirmek istedik.
