An Intramucosal Mucinous Adenocarcinoma Arising From A Gastric Hyperplastic Polyp

Abstract
Hyperplastic polyps constitute 75% of all gastric polyps. Most of them are innocent lesions and are smaller than 1 cm. They develop as a consequence of an exaggerated mucosal response to tissue injury and inflammation on the basis of chronic gastritis. A 69-year-old male was admitted to the Gastroenterology Clinic with complaints of abdomi- nal pain and vomiting. Gastroscopy showed a polyp that was 2 cm in its greatest dimension. A histopathologic examination of this lesion re- vealed adenocarcinoma infiltration in the mucosa. Subtotal gastrectomy was performed. Macroscopically, a polyp with a stalk was seen in the an- trum; microscopically, it was consisted with a hyperplastic polyp. In the polyp, a few foci of infiltrative glands embedded in the mucinous matrix were noticed in the lamina propria, and the patient was diagnosed as in- tramucosal mucinous adenocarcinoma arising from a gastric hyperplas- tic polyp. Because of the risk of carcinoma development, which is rare, all hyperplastic polyps should be completely excised, and a histopathologic examination should be carefully performed

Tüm gastrik poliplerin %75'ini oluşturan hiperplastik polipler genellikle 1 cm'den küçük, masum lezyonlardır. Çoğunluğu kronik gastrit zemininde, doku hasarı ve abartılı mukozal regenerasyon sonucu gelişir. Hiperplastik poliplerde karsinom gelişme riski %2'den az olarak bildirilmiştir. Büyük ve multipl poliplerde risk artar. Burada antrumda lokalize, soliter hiperplastik polip zemininde gelişen intramukozal adenokarsinom olgusu sunulmuştur. 69 yaşında erkek hasta karın ağrısı ve kusma şikayeti ile gastroenteroloji kliniğine başvurdu. Gastroskopide antrumda 2 cm çaplı polip görüldü. Bu lezyondan alınan biyopsinin histopatolojik incelemesinde mukozada adenokarsinom infiltrasyonu saptanması üzerine subtotal gastrektomi uygulandı. Makroskopik olarak antrumda izlenen 2x1 cm ölçüsündeki saplı polibin histopatolojik incelemesinde hiperplastik polip ile uyumlu morfolojik bulgular saptandı. Polip içerisinde, lamina propriada, birkaç küçük odak tarzında, müsinöz zeminde infiltratif glandüler yapılar dikkati çekti ve olgu hiperplastik polip zemininde gelişmiş müsinöz adenokarsinom olarak rapor edildi. Hiperplastik poliplerde nadir de olsa karsinom gelişimi görülebileceğinden biyopsi ile yetinmeyip total olarak çıkarılması ve histopatolojik incelemesinin dikkatle yapılması gerekmektedir.
