Stenting for Severe Vertebral Artery Orifice Stenosis in A Case of Three Cerebral Artery Occlusion

Abstract

A 51-year-old male presented with dysarthria and weakness on his left side. In his history, he had an operation due to aortic coarctati- on 13 years ago, and 5 years ago, he had a transient ischemic attack along with difficulty in speaking. During his neurological examinati- on, he was conscious, cooperative, and oriented. Mild dysarthria and left hemiparesis were seen (4/5, 4/5). He smoked at least one cigarette packet per day for 30 years. Vascularization was performed only by narrowed right vertebral artery on cranial and cervical magnetic re- sonance angiography. He had a mild increase in his left hemiparesis during his stay in the hospital (2/5, 4/5). A stent had been inserted into his right vertebral artery during digital subtraction angiography. In his last neurological examination, he was conscious, cooperative, and oriented. We started prasugrel (10 mg/day) and acetylsalicylic acid (300 mg/day) for his mild dysarthria and mild hemiparesis. In this case, we explain that although there was three-vessel occlusion and narrowing in the right vertebral artery, the patient’s neurologic status was not too bad due to collateral circulation around the cereb- ral arteries. Özet

51 yaşında erkek hasta, konuşma bozukluğu ve sol tarafında güçsüzlük şikayeti ile başvurdu. 13 yıl önce aort koarktasyonu nedeniyle opere edilen ve 5 yıl önce de konuşma bozukluğunun olduğu iskemik atak geçiren hastanın nörolojik muayenesi, şuur açık, koopere, oryente ve hafif dizartrikti. Kas gücü sol hemiparezik 4/5, 4/5 olarak değerlendirildi. Alışkanlıklarında; 30 yıldır günde 1 paket sigara içme öyküsü vardı. Kranial ve servikal manyetik rezonans anjiografide serebral kanlanmanın sadece dar olan sağ vertebral arterden olduğu görüldü. Hastanın yattığı süre içinde sol hemiparezisinde hafif bir kötüleşme oldu (2/5, 4/5). DSA (Digital Subtraction Angiography) sonrasında hastanın sağ vertebral arter orifisine stent takıldı. Son nörolojik muayenesi, şuur açık, koopere, oryente ve hafif dizartrikti. Kas gücü sol hemiparezi 3/5, -5/5 olarak değerlendirildi. Medikal tedavi olarak hastaya prasugrel 10 mg/ gün ve asetil salisilik asit 300 mg/gün başlanarak taburcu edildi. Bu olguda üç damar oklüzyonu olması ve sağ vertebral arterdeki darlığa rağmen nörolojik tablonun bu oranda kötü olmaması serebral arterler arasındaki kollateral dolaşımın iyi olması ile açıklanabilir. Bu olgu sunumu ile serebral kanlanmada kollateral dolaşımın ne kadar önemli olduğunu hatırlatmak istedik.
