Seroprevalance of Rubella, Cytomegalovirus and Toxoplasma Antibodies Among Pregnant Women that Referred to Sorgun State Hospital

Abstract
Objective: Rubella, Cytomegalovirus (CMV) and toxoplasma infections of the fetus can lead to serious congenital birth defects or fetal death. Material and Methods: The aim of this study is to investigate seroprevalance of antibodies against rubella, CMV and toxoplasma that may lead to congenital infection among pregnant women who applied to Sorgun State Hospital polyclinics of gynecology and obstetrics in the period between January 2012 and December 2012. Rubella, CMV and toxoplasma immunglobulin M (IgM) and immunglobulin G (IgG), CMV and toxoplasma IgG avidity antibodies were retrospectively investigated in 804 serum samples, by using the enzyme linked fluorescence method (ELFA; Enzyme Linked Fluorescence Assay, Vidas, Bio Mérieux, France), obtained from the 15-50 year-old pregnant women who applied to the hospital. Results: Seropositivity rates of rubella, CMV toxoplasma IgM antibodies were found to be 0.1, 0.1, and 0.2 %, and those of IgG antibodies were 94.0, 99.8, and 36.9 %, respectively. Conclusion: For the prevention of congenital infections and establishment of a final diagnosis, we recommend concurrent investigation of both IgM and IgG antibodies before and during pregnancy. If both IgM and IgG positivities are detected, using IgG avidity test acute or previous infection should be identified.

Amaç: Fetusun rubella, sitomegalovirus (CMV) ve toksoplazma ile infekte olması ciddi konjenital anomalilere veya ölü doğuma yol açabilmektedir.Gereç ve Yöntem: Bu çalışmada Sorgun Devlet Hastanesi Kadın Hastalıkları ve Doğum Polikliniklerine Ocak 2012 ile Aralık 2012 tarihleri arasında başvuran gebelerde rubella, CMV ve toksoplazma seropozitiflik oranlarının saptanması amaçlanmıştır. Hastaneye başvuran 15-50 yaş arası hamile kadınlardan alınan 804 serum örneğinde enzim floresans yöntemi (ELFA; Enzyme Linked Fluorescence Assay, Vidas, Bio Mérieux) ile bakılan rubella, CMV, toksoplazma Immünglobulin M (IgM) ve Immünglobulin G (IgG), CMV ve toksoplazma IgG avidite antikor kayıtları retrospektif olarak incelenmiştir.Bulgular: Rubella, CMV ve toksoplazma IgM antikorlarının seropozitiflik oranları sırasıyla % 0.1, % 0.1, % 0.2, olarak tespit edilmiş, rubella, CMV ve toksoplazma IgG antikorlarının seropozitiflik oranları ise sırasıyla; % 94.0, % 99.8, % 36.9 olarak bulunmuştur.Sonuç: Gebelerde rubella, CMV ve toksoplazma enfeksiyonu açısından asıl tanı koyduran gebelik öncesi seronegatif iken, gebelikte pozitif sonuç elde edilmesidir. Bu nedenle konjenital enfeksiyonların önlenebilmesi ve doğru tanı konulabilmesi için önerimiz gebelik planlanırken ve gebelikte tekrar antikor durumuna bakılması; IgM ve IgG nin eşzamanlı bakılması; her ikisinin de pozitif olması durumunda IgG avidite testiyle enfeksiyonun akut enfeksiyon mu yoksa geçirilmiş enfeksiyon mu olduğuna karar verilmesidir.
