Effects of Immune-Modulating Enteral Nutrition and Blood Transfusion in Experimental Colonic Anastomosis in Rats

Abstract

Objective: There are few studies in literature evaluating and comparing the effects of immunonutritional support and blood transfusions on colonic anastomotic healing. We aimed to comparatively investigated the effects of blood transfusions and early pre- and postoperative enteral immunonutrition on the experimental colonic anastamosis in rats.Material and Method: The study included 40 female inbred Dawley rats divided in 4 groups. Group 1 control group, anastomosis was done and the rats were fed ad libitum; Group 2 anastomosis was done, the rats were fed ad libitum and blood transfusion was delivered; Group 3 anastomosis was done immunoenteral nutrition was given; Group 4 anastamosis was done, immunoenteral nutrition was given and blood transfusion was delivered. Rats were sacrified on postoperative 7 days after the first laparotomy procedure. Macroscopic findings within the abdominal cavity, and anastamosis region were recorded. Anastomotic bursting pressures were measured, and expressed in mm Hg, and tissue hydroxyproline levels were indicated in µmol/L.Results: Anastomotic bursting pressure was statistically significantly higher in Group 1 than in Group 2 (p0.05).

Özet

Amaç: Literatürde immün nütrisyonel desteğin ve kan transfüzyonlarının kolonik anastomoz iyileşmesi üzerine etkilerini beraber araştıran ve karşılaştıran az sayıda çalışma mevcuttur. Bu çalışmada, ratlarda oluşturulan deneysel kolon anastomozlarını, yapılan kan tranfüzyonları ile preoperatif ve postoperatif erken dönemde verilen enteral immünonütrisyonun etkilerini karşılaştırmalı araştırmayı amaçladık.Gereç ve Yöntem: Kırk adet, dişi, inbred Dawley cinsi rat 4 gruba ayrıldı; Grup 1, kontrol grubu, anastomoz yapılıpnormal besin alan (NBA); Grup 2, anastomoz yapılıpnormal besin alan ve kan transfüzyonu yapılan (NBAT); Grup 3 anastomoz yapılıp immünoenteral nütrisyon alan (İNA); Grup 4, anastomoz yapılıp immünenteral nutrüsyon ve kan transfüzyonu alan (İNAT). Ratlar ilk laparatomiyi takiben, 7 gün sonra sakrifiye edildi. Batın içi ve anastamoz bölgesindeki makroskopik bulgular kaydedildi. Anastomoz hattı patlama basınçları ölçüldü mmHg cinsinden kaydedildi ve doku hidroksiprolin değerleri Umol/gr doku cinsinden ölçüldü.Bulgular: Grup 1'de patlama basıncı değeri, Grup 2'nin değerlerine göre istatistiksel olarak anlamlı derecede daha yüksek bulunmuştur (p0,05).
