Anesthetic Approach to Thyroid Medullary Carcinoma Patient with a Diagnosis of Severe Hemophilia A

Abstract

Hemophilia A is the most frequently observed genetic bleeding disorder with an X-linked recessive trait resulting from the deficiency of factor 8 (F8). F8 deficiency leads to prolonged bleeding episodes after traumas, tooth extraction, and surgical procedures. Surgical procedures in hemophilia A patients present peculiarities. The aim of this study is to discuss principles of preoperative preparation, perioperative methodology and postoperative monitorization concerning neck dissection of the patients with the diagnosis of severe hemophilia A. A 30 year-old, 70 kg male patient presented to the endocrinology clinic with complaint of neck mass which he noticed 6 months ago. Following a detailed examination, diagnosis of thyroid malignancy was made. Medical history of the patient revealed serious bleeding after a tooth extraction when he was 8 years old, spontanous subarachnoid bleeding and 8-10 annual occasions of bleeding into the ankle or muscle. According to the examinations made one week before the operation, aPTZ was 94.3 sec, and F8 activity was 0%. Twelve hours before the operation, oral transemic acid tablet at a dose of 500 mg qid was started and maintained for 10 days. Two days before the operation, 3000 IU F8 was infused through intravenous route and in the perioperative period, replacement therapy with infusions of F8 concentrates was maintained. Administration of F8 concentrate and transemic acid was continued until the end of the postoperative 10th day. Although the targeted F8 activity-level during the preoperative and perioperative periods were not reached, we think that since bleeding complications were not observed, the administration of F8 replacement therapy at an appropriate dosage and time during the postoperative period according to the magnitude of the surgery is more important than reaching the targeted F8.

Hemofili A; faktör 8 (F8) eksikliği sonucu meydana gelen, X'e bağlı resesif geçişli, en sık rastlanan kalıtsal kanama bozukluğudur. F8 eksikliği; yaralanma, diş çekimi ve cerrahi işlemler sonrası uzamış kanamalara ve gecikmiş yara yeri iyileşmesine neden olur. Hemofili A hastalarının cerrahi uygulamaları özellik gösterir. Bu makalede, ağır hemofili A tanılı hastanın boyun diseksiyonu ameliyatının preoperatif hazırlığı, peroperatif yönetimi ve postoperatif takip ilkelerinin tartışılması amaçlandı. Otuz yaşında, 70 kg ağırlığında erkek hasta, 6 ay önce fark ettiği boyunda şişlik yakınmasıyla endokrin polikliniğine başvurmuş ve yapılan ayrıntılı incelemesinde hastaya tiroid malignitesi tanısı konmuş. Özgeçmişinde 8 yaşında diş çekimi sonrası ciddi kanama, 18 yaşında spontan subaraknoid kanama ve yılda 8-10 defa eklem ve kas içine kanama öyküsü mevcut. Operasyondan 1 hafta önce yapılan tetkiklerinde aPTZ 94,3 sn, F8 aktivitesi %0 olarak belirlenmiş. Operasyondan 12 saat önce hastaya transemik asit tablet 4x500 mg dozda oral yoldan başlandı ve 10 gün boyunca devam edilmesi planlandı. Operasyondan 2 saat önce 3000 IÜ F8 konsantresi IV olarak uygulandı ve peroperatif dönemde F8 konsantresi replasman tedavisine devam edildi. F8 konsantresi ve transemik asit uygulaması postoperatif 10. güne kadar sürüdürüldü ve hastada kanama komplikasyonu yaşanmadı. Preoperarif ve peroperatif dönemde hedeflenen F8 aktivitesi seviyesine ulaşmamış olmamıza rağmen, kanama komplikasyonu yaşamadığımız için, ameliyatın büyüklüğüne göre yeterli dozda ve sürede F8 konsantresi replasman tedavisi uygulanmasının, hedef F8 aktivitesi seviyelerine ulaşılmasından daha önemli olduğunu düşünmekteyiz.
