Gastrointestinal Surgical Problems Formed Of Ingested Foreign Bodies

Abstract
Introduction: Foreign body ingestion is common problem encounteredduring childhood. In this study, we aimed to discuss ourclinical experiences with gastrointestinal problems caused byforeign body ingestion in the light of the the literature.Material and Method: All hospital files of the children whowere treated with the diagnosis of foreign body ingestion in twocenters between 1987 and 2013; were analyzed retrospectively.Results: Six (46 %) male, and 7 (54 %) female patients amountingto a total of 13 cases with a history of foreign body ingestionwho were diagnosed, and treated during a period of 26years in two centers were retrospectively analyzed. Mean age ofthe patients was 36.6 months and 10 of them had a history offoreign body ingestion at admission. Three patients had nothistory of foreign body ingestion of evidence of foreign body onradiograms. These 3 patients had required emergency surgeryand waxcloth piece, pine tree leaf and small plastic balls werefound in their bowels. Other 10 patients had required surgerybecause of persistence of the foreign bodies (5 safety pins, 1 pin,1 hairpin, and 1 coin). Foreign bodies were found in the stomach(n=21), duodenum (n=1), Treitz fascia (n=5),ileocecalvalve or ileum (n=4). One of them was removed by rectal touch,12 foreign substances were removed during laparotomy. In onepatient foreing body was pushed into rectosigmoid colon; onehad been removed with appendectomy while 11 of them hadbeen removed with enterotomy. Two patients had mental retardationAll patients had recovered completely, while one of themrequired laparotomy due to brid ileus.Discussion and Conclusion: Foreign body ingestion maycause serious problems in children. The diagnosis and treatmentof foreign body ingestion may be delayed or become difficultif medical history can not be obtained or evidence offoriegn body is not found on radiograms. We think that diagnosisof foreign body ingestion should be kept inmind in caseswith atypical symptoms of acute abdomen.

Amaç: Yabancı cisim yutulması, çocukluk çağının sık karşılaşılan bir sorunudur. Bu cisimler bazen çok ciddi komplikasyonlara yol açabilmektedir. Makalemizdeki amaç, gastrointestinal sistemde yabancı cisimlerin oluşturduğu cerrahi sorunlarla ilgili klinik deneyimlerimizi literatür eşliğinde tartışmaktır. Gereç ve Yöntem: İki merkezde 1987 ile 2013 yılları arasında yabancı cisim yutulması tanı ve tedavisi yapılan çocukların hastane arşivlerinden ulaşılabilen dosyaları geriye dönük olarak incelendi. Bulgular: İki merkezde 26 yıllık süreçte tanı ve tedavisi yapı- lan toplam 13 olgunun 6'sı (%46) erkek, 7'si (%54) kız hasta idi. Yaş ortalaması 36.6 ay (5 ay-9 yaş) olan olguların 10'ununda başvuru sırasında yabancı cisim yutma öyküsü varken, 3'ünde öykü olmadığı gibi, çekilen grafide de yabancı cisime rastlanılmadı. Bu üç olguda gelişen akut karın tablosu sonrası yapılan ameliyatta: muşamba parçası, çam ağacı yaprağı ve küçük plastik top saptandı. Diğer 10 hastada ise 5 çengelli iğne, 1 toplu iğne, 1 çivi, 1 vida, 1 saç tokası ve 1 madeni para aynı yerde sebat ettiğinden girişim gerektirdi. Yabancı cisimlerin 2'si midede, 1'i duodenumda, 5'i Treitz'de, 4'ü ileoçekal valv veya ileumda, 1'i de rektumda saptandı. Yabancı cisimlerin biri rektal tuşe ile çıkarılırken on ikisinde laparatomi gerekti. Birinde yabancı cisim rektosigmoide itildi, birinde apendektomi, 11'inde ise enterotomi ile yabancı cisim çıkarıldı. İki olguda mental gerilik mevcuttu. Şifa ile taburcu edilen hastalardan birinde 20 gün sonra bridektomi yapılması gerekti. Tartışma ve Sonuç: Yutulan yabancı cisimler çocuklarda ciddi sorunlara neden olabilmektedir. Öykü alınamayan veya grafide saptanamayan yabancı cisimlerin tanı ve tedavisinde gecikmeler olabilmektedir. Atipik seyirli akut karın bulguları olan olgularda altta yatan bir yabancı cisim olabileceği akılda tutulmalıdır.
