Retrospective Analysis of the Effects of Anesthesia Methods Applied in Cesarean Section Cases on Changes in Apgar Scores of the Newborn

Abstract

Objective: Cesarean section is the most important procedure in obstetric surgery. A normal surgical anesthesia attempts to ensure the safety and optimal conditions of only one person; while in obstetric anesthesia safety of both mother and the fetus which is affected by every change in the mother should be secured.Material and Method: Anesthesia management and neonatal records of 300 pregnant women undergoing cesarean operation between January 2016-June 2016 were retrospectively screened. Demographic data of the patients, number of pregnancies, gestational age, parity, emergency/elective cesarean section, anesthesia method, accompanying diseases, birth weight of the newborn, 1. and 5. minute Apgar scores were recorded.Results: It was observed that the duration of the operation was longer and the average 1st and 5th minute Apgar score was higher at procedures with spinal anesthesia. Statistically higher number of pregnancies, and increased newborn weight were observed in patients who underwent spinal anesthesia. It was also observed that 86.5% of the emergency cases were performed under general, and 55% of elective cases under spinal anesthesia with a statistically significant intergroup difference.Conclusion: In caesarean section cases, spinal anesthesia for mother is preferred in terms of comfortable postoperative period and in consideration of the first minute Apgar score in pregnancies bearing fetal stress risk. In this study, we found that cesarean section by spinal anesthesia is a positive effect on the neonatal Apgar score.

Özet

Amaç: Obstetrik cerrahide en önemli girişim sezaryendir. Sezaryende uygulanan anestezinin rutin anestezi uygulamalarından farkı, bir kişinin güvenliğini ve optimal koşullarını sağlamak yerine, annenin ve annede gelişen her türlü olaydan etkilenen fetusun da güvenliğinin sağlanmasıdır.Gereç ve Yöntem: Ocak 2016-Haziran 2016 tarihleri arasındaki dönemde sezaryen operasyonu geçiren 300 gebenin anestezi yönetimi ve yenidoğan kayıtları retrospektif olarak incelendi. Hastaların demografik verileri (yaş), gebelik sayısı ve haftası, paritesi, sezaryenin acil ya da elektif olması, yandaş hastalıkları, uygulanan anestezi yöntemi, yenidoğanın ağırlığı, yenidoğan 1. ve 5. dk. Apgar skorları kaydedildi. Bu yöntemlerin anne ile yenidoğan üzerine etkileri değerlendirildi.Bulgular: Spinal anestezi uygulananlarda ameliyat süresinin daha uzun sürdüğü ve ortalama 1. ve 5. dk. apgar skorunun daha yüksek olduğu gözlendi. Gebelik sayısının ve yenidoğan ağırlığının spinal anestezi uygulananlarda istatistiksel olarak daha fazla olduğu belirlendi. Acil olarak alınan gebelerin %86,5'i genel; elektif olarak alınan gebelerin %55'i genel anestezi, %45'i spinal anestezi uygulandığı ve istatistik olarak anlamlı bulunduğu gözlendi. Sonuç: Sezaryen olgularında, anne için spinal anestezi postoperatif dönemin konforlu geçmesi açısından ve fetal stres riski taşıyan gebeliklerde ilk dakika Apgar skoru dikkate alınarak tercih edilir. Bu çalışma ile spinal anestezi uygulanarak sezaryen yapılmasının yenidoğan Apgar skoruna olumlu etkisi olduğunu tespit ettik.
