Can Chlamydia trachomatis-IgG Serology be Used for Diagnosing Ectopic Pregnancies and Predicting Treatment Outcome?

Abstract
Objective: To define if Chlamydia trachomatis-IgG seropositivity can discriminate ectopic pregnancy cases from women with normal reproductive histories; and whether Chlamydia trachomatisIgG seropositivity can predict therapeutical success when medical management or monitorization were decided upon.Material and Methods: In a prospective design, 3 patient groups were formed, (Group 1): patients diagnosed as ectopic pregnancy and succesfully treated with methotrexate; (Group 2): failed with methotrexate and surgically treated; (Group 3): the control group consisted of healthy patients without history of ectopic pregnancies. Sera of all blood sample were isolated and analysed with the double sandwich ELISA technique to semiquantitatively measure IgGChlamydia trachomatis-IgG concentrations. Three groups were compared as for the presence of ectopic pregnancy, and results of serologic tests.Results: Eighty-nine cases were included in the study: (Group 1: 30; Group 2: 29; Group 3: 30). The mean age in Group 2 was significantly older than that of Group 3 (31.5±1.3 and 27.4±1) and similar to Group 1. The mean number of abortions in Group 1 was higher than Group 3 (0.6±0.2 and 0.03±0.03). Free peritoneal fluid was observed at a higher rate in Group 2 than Group 3. Adnexial masses were observed in a similar number of cases in Groups 1 and 2 which were, significantly higher than Group 3. The ?-hCG concentrations were significantly higher in Group 2 than in Group 1 (5917.3±945.5 mIU/ml, and 1493.3±359.5 mIU/ml, respectively). The Chlamydia trachomatis IgG positivity did not differ significantly among Groups 1,2 and 3 (46.7%, 44.8% and 33.3i respectively); and between Groups 1 + 2 and Group 3 (45.8% and 33.3).Conclusion: Chlamydia serology (Chlamydia trachomatis-IgG) can not be used to discriminate cases with ectopic pregnancies from healthy cases; and also it can not differentiate cases successfully treated with methotrezate from patients whose surgical treatment is needed.

Amaç: Chlamydia trachomatis-IgG seropozitifliğinin ektopik gebelik olgularını normal reprodüktif özgeçmişi olan olgulardan ayırd ediciliği var mıdır? Medikal tedavi veya izlem kararı verilmiş olgularda Chlamydia trachomatis-IgG seropozitifliği başarıyı öngörebilir mi?Gereç ve Yöntem: Hasta grupları prospektif olarak, ektopik gebelik tanısı almış ve metotrexat tek doz tedavisi ile başarılı olunmuş (Grup 1) veya metotreksat ile tedavisi başarısız olmuş ve opere edilmiş olgulardan oluşturuldu (Grup 2). Kontrol grubunu oluşturan olguların (Grup 3) özgeçmişlerinde ektopik gebelik yoktu. Tüm olgulardan kan örnekleri serumları ayrıştırılıp, seroloji laboratuvarında çift-antijen sandwich tekniği ile ELISA testi kullanılıp, serumlarda IgG-Chlamydia trachomatis ölçümleri semi-kantitatif olarak yapıldı. Üç grup demografik, ektopik gebelik bulguları ve seroloji sonuçları açısından karşılaştırıldı.Bulgular: Çalışmaya 89 olgu dâhil edildi: (Grup 1: 30; Grup 2: 29; Grup 3: 30). Grup 2 hasta ortalama yaşı anlamlı olarak Grup 3 ortalama hasta yaşından daha yüksekti (31,5±1,3 ve 27,4±1), Ortalama abortus sayısı Grup 1'de Grup 3'ten daha yüksekti (0,6±0,2 ve 0,03±0,03). Serbest peritoneal sıvı varlığı Grup 2'de Grup 3'ten daha yüksek oranda mevcuttu. Adneksiyal kitle varlığı Grup 1 ve 2 arasında fark yoktu. Grup (1 ve 2) ile Grup 3 arasında anlamlı bir fark mevcuttu. Grup 2 kan ?-hCG konsantrasyonları Grup 1 olgularından anlamlı olarak daha yüksekti (1493,3±359,5 mIU/ml ve 5917,3±945,5 mIU/ml). Chlamydia trachomatis IgG pozitiflik oranları: Grup 1, 2 ve 3 arasında (%46,7, %44,8 ve %33,3) ve (Grup 1+Grup 2) vs Grup 3 karşılaştırmasında anlamlı farklılık göstermedi (%45,8 ve %33,3).Sonuç: Chlamydia serolojisi dış gebelik olgularını dış gebelik hiç geçirmemiş olgulardan veya metotreksat tedavisinde başarılı olunmuş olguları cerrahi ile tedavi gereği duyulmuş olgulardan ayırt edici değildir.
