Abstract
Promising improvements provided with the modern patient care approaches in ERAS implementations, which have started with colorectal surgery, have been extended from laparoscopy to bariatric surgery, and from hepatopancreaticobiliary surgery to breast reconstruction. The ERAS implementations provided the control of the care process re-experienced by the patient and health practitioners, through the use of evidence-based measurements, sampling and teaching of the traditional care. While shorter hospital stays were achieved in all disciplines, privileged disciplinary new approach modalities have been also developed. Preoperative weight loss, restriction of alcohol, and cigarette use, differences in approach to fluid and respiratory physiology in bariatric surgery, reduction of opioid use in breast surgery, evaluation of requirements for biliary drainage in pancreatic tumor surgery etc. are some examples. Currently, in all general surgical disciplines there is a need for further studies in which new methodologies and implementations will be evaluated in order to control and question the process.
Özet
Kolorektal cerrahi ile başlayan ERAS uygulamalarındamodern hasta bakımı yaklaşımı ile sağlanan ümit vericigelişmeler laparoskopiden, bariatrik cerrahiye, hepatikopankreatikcerrahi girişimlerden meme rekonstrüksiyonunauzanmıştır. ERAS uygulamaları geleneksel yaklaşımınkanıta dayalı değerlendirmelerle ölçülmesi, örneklenmesive öğreti yoluyla hasta ve sağlık uygulayıcılarının yenidendeneyimlediği bakım sürecinin kontrolünü sağlamıştır. Tümdisiplinlerde daha kısa yatış süreleri elde edilirken, disiplinayrıcalıklı yeni yaklaşım modaliteleri de gelişmiştir.Bariatrik cerrahide operasyon öncesi kilo kontrolü, alkolsigarakullanımı kısıtlamaları, sıvı ve solunum fizyolojisineyaklaşım farklılıkları, meme cerrahisinde opioid kullanımınınazaltılması, pankreas tümörlerindeki girişimlerde biliyerdrenaj gereksiniminin değerlendirilmesi vb. gibi farklılıklarıgetirmiştir. Hâlen tüm genel cerrahi disiplinlerdesürecin kontrolü ve sorgulanması amacıyla yeni metodolojive uygulamaların değerlendirildiği çalışmalara gereksinimvardır.
