ERAS Protocols in Anesthesiology and Perioperative Medicine, ERAS and Anesthesia

Abstract
Enhanced Recovery After Surgery (ERAS) which was firstly revived with the studies performed by Henrik Kehlet during 1990s, is a concept that describes patientcentered, evidence-based perioperative interventions. This concept is managed by multidisciplinary teams that include various areas of expertise to minimize the patient’s surgical stress response, optimize physiological functions, and facilitate healing. These concepts include stages of the patient care from home to hospitalisation, preoperative, intraoperative and postoperative phases of surgery, and discharge. With the use of ERAS protocols, perioperative and postoperative complications decreased, patient survival and quality of care improved, and patient satisfaction was significantly increased. In order to further increase these developments in various surgical specialties, protocols have been established on this subject and many health institutions offer their services in this way. The aim of this review is to reveal the evidencebased medical information based on the current literature and protocols related to the subject.
Özet
İlk olarak 1990’lı yıllar da Henrik Kehlet’in yaptığı çalışmalarile gündeme gelen Enhenced Recovery After Surgery(ERAS) hasta odaklı, kanıta dayalı tıp kaynaklı, perioperatifmüdahaleleri tanımlayan kavramdır. Bu kavram hastanıncerrahi stres yanıtını azaltmak, fizyolojik fonksiyonlarınıdüzeltmek ve iyileşmeyi kolaylaştırmak amacıyla çeşitliuzmanlık alanlarını içeren multidispliner ekipler tarafındanyönetilir. Bu hasta bakım ve müdahale konseptleri birbütün halinde hastanın evden çıkışı, hastaneye yatışı, cerrahininpreoperatif, intraoperatif ve postoperatif fazlarınıayrıca hastanın eve taburculuğuna kadar geçer süre veevreleri oluşturur. ERAS protokollerinin kullanımı ile periopetifve postoperatif komplikasyonlarda azalma, hasta sağkalımında ve bakım kalitesinde artış ve hasta memnuniyetindebelirgin gelişmeler sağlanmıştır. Günümüzde değişikcerrahi uzmanlık alanlarında bu gelişmeleri daha da arttırmakamacıyla bu konu ile ilgili protokoller oluşturulmuşve birçok sağlık kuruluşu hizmetlerini bu kavrama dayalıbir şekilde sunmaktadırlar. Bu çalışmanın amacı konu ileilgili güncel literatür ve protokoller ile ortaya çıkan kanıtadayalı tıbbi bilgilerin ortaya konmasıdır.
