Predictive Factors in Incidental Thyroid Carcinoma: A Retrospective Study

Abstract

Introduction: The incidence of incidental thyroid carcinoma has increased in recent years but despite of that, no specific features has been established for prediction of these cases. In this study, we aimed to investigate whether there are specific features for incidental thyroid cancer. Methods: Patients who underwent thyroidectomy for nodular goiters due to benign or malignant indications were divided into 3 groups. Groups were classified as Group I: Patients without any clinical and radiological evidence of cancer risk, but operated due to their symptoms, and histo-pathological examination of their surgical specimens was reported as benign lesion: Group II: Patients whose clinically and radiological examination did not suggest any risk of cancer, and operated due to their symptoms, and histopatholoic examination of their surgical specimens revealed the presence of malignancy (incidental group); Group III: Patients whose FNAB was reported as malignant/suspicious, thus operated with the diagnosis/prediagnosis of malignancy, and their thyroid histopathology was reported as malignant In addition, in Groups I and II FNAB done from nodule with highest risk of malignancy potential before surgery, and the outcome of FNAB was reported as benign cytology. Results: There were 51 cases (51%) in Group I, 16 (16%) in Group II and 33 (33%) in Group III. There was no difference in average age between Group II and Group I and Group III (p>0.05). There was a difference in sex between Groups II and III (p=0.025). According to ultrasonographic findings whether nodule diameter was greater than 20 mm was correlated between Group II and Group III (p=0.005). Conclusion: In especially male patient with multiple thyroid nodules over two centimeters, biopsy should be done not only from nodule with malignancy potential but also from other nodules.

Özet

Amaç: İnsidental tiroid karsinomu son yıllarda artış göstermekte ancak bu artışa rağmen bu olguların saptanması için henüz belirlenmiş spesifik özellikler ortaya konamamıştır. Çalışmamızda insidental tiroid kanseri için spesifik özellik olup olmadığını araştırmayı amaçladık.Gereç: Benign ya da malign nodüler guatr nedeniyle tiroidektomi uygulanan olgular 3 gruba ayrıldı. Olgular; Grup I: Klinik ve radyolojik olan malignite riski olmayan ancak semptomları nede-niyle ameliyat edilip ameliyat histopatolojisi benign gelenler; Grup II: Klinik ve radyolojik olarak malignite düşünülmeyip semptomları nedeniyle ameliyat edilip ameliyat histopatolojisinde malignite görülenler (İnsidental grup), grup III: preoperatif ince iğne aspirasyon biyopsi (İİAB)’si malign ya da kuşkulu olup malignite tanısı/ön tanısıyla ameliyat edilip tiroidektomi histopatoloji-si malign gelenler olmak üzere 3 gruba ayrıldı. Ek olarak grup I ve grup II’ye ameliyat öncesi radyolojik olarak en riskli nodülden İİAB yapıldı ve İİAB sonucu benign sitoloji idi.Bulgular: Grup I’de 51 (%51), Grup II’de 16 (%16) ve Grup III’de ise 33 (%33) olgu mevcuttu. Grup İİile grup I ve grup III yaş ortalaması açısından fark yoktu (p>0.05). Cinsiyet açısından Grup II ile Grup III arasında farklılık görüldü (p=0.025). Ultrasonografi bulgularına göre nodül çapının 20 mm’nin üzerinde olup olmaması Grup II ve Grup III arasında ilişkili bulundu (p=0.005).Sonuç: İki cm üzeri birden çok tiroid nodülü olan özellikle erkek olgularda sadece malignite potan-siyeli olan nodülden değil aynı zamanda diğer nodüllerden de biyopsi yapılmalıdır.
