Imaging Findings and Long-Term Treatment Outcomes in Coats’ Disease

Abstract
Objective: Our aim was to evaluate the imaging findings and treatment outcomes of patients diagnosed with Coats’ disease in our clinic. Method: Records of patients diagnosed with Coats’ disease in our clinic between 2010 and 2016 were evaluated retrospectively. Stage of the disease, imaging findings and clinical characteristics at the time of diagnosis and at the final visit, and the treatment received were noted. Records of fundus photos, fundus florescein angiography and optic coherence tomography (OCT) imagings were reviewed. The relationship between visual acuity (VA) and central macular thickness (CMT) was analyzed. Results: Thirteen eyes of 13 patients were included in this study. Four patients were over 18 years of age. Ten patients received combined treatment of bevacizumab and laser photocoagulation, whereas 4 patients underwent surgery. Intravitreal dexamethasone implant was applied to 2 patients. OCT images of eleven patients were available, and symptoms of 9 patients regressed, of 1 patient progressed, and 1 patient remained stable in terms of OCT findings. Any correlation was not found between initial and final CMT and VA (p=0,89, r=0,47). Conclusion: Coats’ disease require regular follow-up and OCT is a useful tool for evaluating the course of disease. Different treatment modalities might be effective for anatomic success and integrity of the globe is preserved, even though functional success is harder to achieve.

Amaç: Kliniğimizde Coats’ hastalığı tanısı ile takip ve tedavi ettiğimiz hastaların görüntüleme bulguları ve tedavi sonuçlarını değerlendirmek. Yöntem: 2010-2016 yılları arasında kliniğimizde Coats’ hastalığı tanısı alan hastaların dosyaları retrospektif olarak incelendi. Hastaların başvuru anındaki evreleri, ilk ve son görüntüleme ve muayene bulguları ve takip süresi boyunca aldıkları tedaviler kaydedildi. Hastaların kayıtlarında bulunan fundus fotoğrafları, fundus florescein anjiografi ve optik koherens tomografi (OKT) görüntüleri incelendi. Görme keskinliği (EİDGK) ve santral maküla kalınlıkları (SMK) arasındaki ilişki incelendi. Bulgular: Çalışmaya 13 hastanın 13 gözü dahil edildi. Dört hasta 18 yaş üzeriydi. On hastada bevasizumab ve lazer fotokoagülasyon kombine tedavisi uygulanırken dört hastada cerrahi tedavi uygulandı. İki hastada intravitreal deksametazon implant uygulandı. On bir hastanın OKT görüntüleri elde edildi ve 9 hastanın bulgularında gerileme ve 1 hastada ilerleme saptanırken 1 hastada değişiklik olmadı. Başlangıç ve son kontrol SMK ve EİDGK değişimleri arasında korelasyon saptanmadı (p=0,89, r=0,47). Sonuç: Coats’ hastalığının düzenli takibi büyük önem taşır ve hastaların takibinde OKT yararlı bir araçtır. Farklı tedavi yöntemleri ile fonksiyonel başarı sağlanamasa dahi anatomik başarı sağlanabilmekte ve glob bütünlüğü korunabilmektedir.
