Is There a Relationship Between Sleep Positions and Carpal Tunnel Syndrome? A Cross-Sectional Study

Abstract
Objective: The purpose of this study is to investigate the presence of the contribution of sleeping positions to the etiology of carpal tunnel syndrome (CTS) in Turkish population. Method: In our study, 157 consecutive adult patients operated for CTS were compared with 178 volunteers without complaints. In both groups, age, gender, number of pregnancies, additional diseases, regularly used drug,s occupation, the mostly preferred sleeping positions and whether the hand or arm is under the head/body were questioned. In addition, the duration of the complaints of CTS patients, how many times they woke up in the night due to CTS complaints, the hand to be operated, and the severity of CTS based on the result of electromyography (EMG) were noted. Results: The average age was higher in the operated group. Women were found riskier for CTS. The rate of CTS was higher in patients with diabetes, hypertension and hypothyroidism. According to the results of EMG results, the patients had moderate (22%), severe ( 60%), and very severe (18%) CTS. The position of the head, arms, and legs during sleep was not a risk factor for CTS. In CTS patients, the trunk was found to be on the side of the affected hand (p=0.009). Amaç: Bu çalışmada, toplumumuzda hastaların tercih ettiği yatış pozisyonlarının karpal tünelsendromu (KTS) oluşumuna olası katkısı araştırılmıştır.Yöntem: Kliniğimizde KTS nedeniyle opere edilen 157 ardışık erişkin hasta ile çalışma grubuve hiç yakınması olmayan sağlıklı 178 erişkin gönüllü ile kontrol grubu oluşturuldu. Her ikigrupta yaş, cinsiyet, gebelik sayısı, ek hastalıkları, düzenli kullandıkları ilaçlar, meslek, en çoktercih ettikleri yatış pozisyonları (baş, vücut, kollar, bacaklar) ve elin ya da kolun başın/vücudun altında olup olmaması sorgulandı. Ayrıca KTS hastalarının yakınma süresi, gecede KTSyakınmalarına bağlı olarak kaç kere uyandıkları, hangi elinden opere olacağı ve elektromiyelografi (EMG) sonucundaki KTS şiddeti not edildi.Bulgular: Ameliyat edilen grupta yaş ortalaması daha yüksekti. Kadınlar KTS için daha risklibulundu. Diyabet (DM), hipertansiyon (HT), hipotiroidi hastalarında KTS oranı daha fazlagörüldü. Sedatif ilaç kullanımı ve meslek ile KTS arasında ilişki kurulamadı. Gebelik sayısındaartış KTS için risk faktörüydü. Hastaların EMG sonuçlarında %22’sinin orta, %60’ının ağır ve%18’inin çok ağır şiddette KTS olduğu görüldü. Baş, kollar ve bacakların pozisyonu KTS içinrisk faktörü değildi. Gövdenin KTS hastalarında etkilenen elin tarafına doğru yan yatar şekilde olduğu belirlendi (p=0.009).
