Risk factors associated with hepatitis B seroprevalence in brothel workers

Abstract
Prostitutes may constitute a risk group for transmission of Hepatitis B Virus (HBV) infections in Turkey. The aim of this study is the evaluation of the HBV infection risk of prostitution associated with history of other possible risk factors in prostitutes as well as in other workers in Edirne Brothel. HBV markers were studied by a makro ELISA method in the sera of 72 prostitutes and 42 other workers in the Edirne Brothel. In prostitutes the positivity of antiHBc, HBsAg and antiHBs were found 60 %, 8 % and 39 %, respectively. These rates showed no statistical difference from other workers. AntiHBc positivity showed statistically significant relation to age and the profession period in prostitutes (p=0.0135and p=0.003628, respectively). The presence of other risk factors excluding sexual intercourse increased the antiHBc positivity from 55 % to 77 % for transfusion, from 50 %to 69% for surgical procedures, from 53 %to 79% for three or more delivery and from 50 %to 63 % for curettage, but these differences were not statistically significant (p>0.05). The rate of condom use as a contraceptive method was only 4 % in prostitutes. So the vaccination is important in these groups for HBV infection that is the only Preventable sexually transmitted disease by vaccine. Since the age and the period of profession increased significantly the nsk of hepatitis B infection in prostitutes, the screening and vaccination should be offered in young ages and in the beginning of the profession.

Türkiye'de Hepatit B Virüsü (HBV) yayılımında rolü olabilecek gruplardan birisi genel kadınlardır. Bu çalışmada Edirne Genelevi'nde çalışan genel kadınlarda ve aynı ortamda bulunan diğer genelev çalışanlarında olası risk faktörleri öyküsü ile birlikte rastgele cinsel ilişkinin getirdiği HBV infeksiyonu riskinin değerlendirilmesi amaçlanmıştır. Edirne Genelevi'nde çalışan 72 genel kadın ve 42 diğer çalışanın serumlarında hepatit B göstergeleri makro ELISA yöntemiyle incelenmiştir. Genel kadınlarda antiHBc % 60, HBsAg % 8, antiHBs % 39 pozitif bulunmuştur. Diğer çalışanlarda bu oranlar istatistiksel olarak anlamlı bir fark göstermemiştir. Genel kadınlarda antiHBc pozitifliği yaşa ve meslek yılına göre istatistiksel olarak anlamlı bir artış göstermiştir (sırasıyla p=0.0135 ve p=0.003628). Cinsel ilişki dışı diğer risk faktörlerinin öyküde varlığı antiHBc pozitifliğinin sıklığını kan transfüzyonu için % 55'den % 77'ye, ameliyat için % 50'den % 69'a, üç veya daha fazla doğum için % 53'ten % 79'a ve kürtaj için % 50'den % 63'e yükseltmesine karşın bu artışlar istatistiksel olarak anlamlı bulunmamışlardır (p > 0.05). Genel kadınlarda korunma yöntemi olarak kondom kullanımı % 4 düzeyindedir. Bu nedenle aşı ile korunulabilir tek cinsel temasla bulaşan hastalık olan hepatit B infeksiyonu için bu grupta aşılama öncelik kazanmaktadır. Yaş ve meslekte geçirilen süre riski anlamlı düzeyde arttırdığına göre genel kadınlarda genç yaşta ve yeni işe başlarken tarama ve sonucunda aşılama önerilmelidir.
