Akut myeloblastik lösemili bir olguda sitomegalovirus antijenemisinin saptanması

Abstract
A 26-year-old male patient who was on high dose ARA-C chemotherapy for acute myeloblastic leukemia (AML) developed retinitis suggestive of Cytomegalovirus (CMV) infection. The CMV antigenemia was positive in two specimens prepared from the same heparinized blood sample (the number of positive cells were 1/2 x 10s and 2/2 x 10s leukocytes) and CMV was isolated from blood and urine by centifugation cell cultures simultaneously. It is concluded that evaluation of CMV antigenemia is important in febrile neutropenia and even to see only one positive cell may be significant in this group of patients.

Akut myeloblastik lösemi (AML) nedeni ile yüksek doz ARA-C tedavisi alan 26 yaşında bir erkek hastada Sitomegalovirus (CMV) infeksiyonu ile uyumlu retinit saptandı. Febril nötropenisi olan hastanın heparinli kan örneğinden hazırlanan iki cytospin preparatında CMV antijenemisi pozitif olarak bulundu (pozitif hücre sayısı 1/2 x 105 PMNL ve 2/2 x 105 PMNL). Eş zamanlı olarak alınan kan ve idrar örneklerinde hızlı hücre kültürü yöntemleri ile CMV izole edildi. Febril nötropenisi olan hastalarda CMV antijenemisi saptanmasının önemli ve bu grup hastalarda cytospin preparatında yalnızca bir pozitif hücre görülmesinin bile anlamlı olduğu sonucuna varıldı.
