Varicella and zoster infections in patients with cancer

Abstract

Varicella and zoster infections are viral diseases with a benign course, causing morbidity and mortality, especially due to central nervous system complications in immunocompromised patients.In this study, the course of varicella and zoster infections in 25 patients with malignancy, eight girls and 17 boys, with age range of 1-15 years hospitalized at Dr. Behçet Uz Children's Hospital, complications of the disease, the side effects and efficacy of acyclovir on the course and prognosis of the disease were evaluated. Eighteen patients had varicella and seven patients zoster. The diagnosis was made only clinically. Fourteen patients had acute lymphoblastic leukemia. One patient had active disease, but the others were in remission when they were infected. Six patients were accompanied by complications. These were leukopenia in five cases and encephalitis in one case. Acyclovir used in therapy regimen showed a good effect on the course of the disease and no side effects due to the drug were encountered.

Özet

Suçiçeği ve zoster infeksiyonları genelde selim gidiş gösteren, ancak immun yetmezliği olan hastalarda, suçiçeği seyrinde özellikle santral sinir sistemi komplikasyonu ile sekel ve mortaliteye neden olabilen viral hastalıklardır. Bu çalışmada 1.1.1990 ile 30.6.1996 tarihleri arasında Dr. Behçet Uz Çocuk Hastanesi'nde suçiçeği ve zona zoster tanıları ile yatırılarak tedavi edilen, yaşları 1 ile 15 arasında değişen, sekizi kız, 17'si erkek toplam 25 kanserli olguda hastalığın seyri, uygulanan asiklovrin hastalığın seyrine ve prognozuna etkisi ile ilacın yan etkileri retrospektif olarak değerlendirildi. Onsekiz olgu suçiçeği, yedi olgu zona tanısı almışdı. Tanı herhangi bir laboratuvar testi kullanılmadan sadece klinik olarak konuldu. Hastaların % 56'sını akut lenfoblastik lösemili olgular oluşturmaktaydı ve bu olguların tümü remisyonda idi. Hastalık 19 olguda komplikasyonsuz seyredip şifa ile sonuçlanırken, altı olguda komplikasyon gelişti. Bunlardan birisinde serebral ve serebellar tutulum gösteren ansefalit tablosu vardı. Tedavide kullanılan asiklovir tüm hastalarda hastalığın seyrine olumlu etki yaparken ilacın yan etkilerine rastlanmadı.
