In vitro susceptibility of salmonella and shigella species to various antibiotics

Abstract

The aim of this paper was to determine the in vitro antibiotic susceptibility of Salmonella and Shigella strains isolated from clinical specimens. From 1991 to 1994 the in vitro antibiotic susceptibility of 127 Shigella species isolated from stool and 147 Salmonella species from blood and stool specimens were investigated at the Laboratory of Bacteriology, Clinical Bacteriology and Infectious Diseases Department of Ankara University. Of the isolated Shigella strains 7.09 % was Shigella dysenteriae. 44.09 % S. flexneri. 8.66 % S. boydii and 40.16 % S. sonnei. Percentages for the Salmonella strains were 2.72 % group A (Salmonella paratyphi A), 52.38 % group B (S. paratyphi B, &. tvohimurium. and other group B Salmonella species not identified). 1.36 % group C fS. aratyphi C) and 43.54 % group D fS. typhi. S. enteritidıs and other group D Salmonella species not identified). All of the Shigella and Salmonella species tested with the antibiotic discs using Kirby-Bauer disc diffusion method, the susceptibility rates were found for ampicillin as 76.7-90 %, ampicillin+sulbactam 85.3-94.5 %, trimethoprim-sulfamethoxazole 60.5-90.8%, chloramphenicol 77.2-89.6%, ofloxacin 100-99 %, ciprofloxacin 97.6-100 %, cefoperazone 96.3-93.1%, and ceftriaxone 100-91.6%, respectively. The susceptibility of all Salmonella spp. against +sulfamethoxazole were similiar to other studies performed in Turkey, but in Salmonella spp. the resistance rates were lower than those of Shigella spp.. The result showed that before therapeutic approaches to dysentery and salmonellosis antimicrobial susceptibility testing should be made and empirical treatment in such infections should include newly improved and more effective antibiotics.

Salmonella ve Shigella türlerinin son yıllarda .antibiyotiklere duyarlılık durumunu saptamak amacıyla bu çalışma yapılmıştır. 1991-1994 yılları arasında Ankara Üniversitesi Tıp Fakültesi Klinik Bakteriyoloji ve İnfeksiyon Hastalıkları Anabilim Dalı Bakteriyoloji Laboratuvarı'nda dışkı kültürlerinden izole edilen 127 Shigella. kan ve dışkı kültürlerinden izole edilen 147 Salmonella cinsi bakteri, tiplendirilerek çeşitli antibiyotiklere karşı duyarlılıkları disk difüzyon yöntemi ile araştırıldı. Shigella'ların % 7.09'u S. dysenteriae. % 44.09'u S. flexneri. % 8.66'sı S. boydu ve % 40.16'sı S. sonnei olarak bulundu. Salmonella'ların % 2.72'si A grubu (S. paratyphi A), % 52.307 B grubu (S. paratyphi B, £ typhimurium. kesin tip saptaması yapılamamış B grubu Salmonella). % 1.36'sı C grubu (S. paratyphi C) ve % 43.54'ü D grubu (S. typhi. S. enteritidıs. kesin tip tayini yapılamamış D grubu Salmonella) olarak bulundu. Kirby-Bauer disk difüzyon metoduyla incelenen Shigella ve Salmonella cinsi bakteriler sırasıyla, ampisiline % 76.7-90, ampisilin+sulbaktama % 85.3-94.5, trimetoprim-sulfametoksazole % 60.5-90.8, kloramfenikole % 77.2-89.6, ofloksasine % 100-99, siprofloksasine % 97.6-100, sefoperazona % 96.3-93.1 seftriaksona % 100-91.6 oranlarında duyarlı bulundu; Salmonella'lar ayrıca sefoperazon+sulbaktam'a % 100 duyarlı bulundu. Shigella'larda ampisilin, trimetoprim-sulfametoksazol ve kloramfenikol direnci, ülkemizde yapılan diğer çalışmalarla uyumluluk gösterdi. Bakteriyoloji laboratuvarında izole edilen Salmonella 'lardaki direnç oranları shigellalardaki direnç oranlarıyla karşılaştırıldığında bu üç antibiyotik için daha az oranlarda direnç gözlendi. Tüm bu bulgular, dizanteri ve salmonelloz tedavisinde antibiyotik duyarlılık testlerinin dikkate alınması ve özellikle bu tür infeksiyonların empirik tedavisinde daha yeni ve etkin antibiyotiklerin uygulanması gerektiğini göstermektedir.
