Investigation of the in vitro activity of cefepime using E-test in nosocomial isolates of gram-negative bacteria

Abstract
Cefepime is a new cephalosporin with a broader antimicrobial spectrum. The unique features of cefepime are more rapid penetration into Gram-negative bacteria, targeting at multiple essential penicillin-binding proteins and resistance to inactivation by many ß-lactamases due to the low affinity of these enzymes for the drug. The aim of this study was to determine the susceptibility of nosocomial Gram-negative bacteria to cefepime. At the Akdeniz University Hospital, a total of 279 nosocomial isolates of Gram-negative bacteria were tested for their antimicrobial susceptibility to cefepime isolates Were analyzed with E-test strips to determine their minimal inhibitory concentrations (MICs). Eighty (28.7 %) Pseudomonas spp, 58 (20.8 %) Acinetobacter spp, 47(16.8%) Escherichia coli 37 (13.3 %) Klebsiella spp, 33 (11.8 %) Enterobacter spp and the other Gram-negative bacteria (8.6 %) were tested. A total of 44.8 % of Acinetobacter spp, 22.5 % Pseudomonas spp, 13.5 % Klebsiella spp, 9.1 % Enterobacter spp and 4.2 % Escherichia coli were found resistant to cefepime.

Sefepim; Gram-negatif basillere daha hızlı penetrasyon, çok sayıda penisilin-bağlayan proteinlere bağlanabilme, çeşitli ß laktamazlarla inaktivasyona karşı direnç gibi özellikleri olan geniş antimikrobiyal spektruma sahip yeni bir sefalosporindir. Akdeniz Üniversitesi Tıp Fakültesi Hastanesi'nde, hastane infeksiyonu etkeni olarak izole edilen 279 Gram-negatif bakterinin sefepime karşı duyarlılıklarının saptanması amaçlandı. E-test ile minimum inhibitor konsantrasyon (MIC) değerleri araştırıldı. Etkenlerin 80'ini Pseudomonas türleri (%28.7), 58'ini Acinetobacter spp (%20.8), 47'sini Escherichia coli (%16.8), 37'sini Klebsiella türleri (%13.3), 33'ünü Enterobacter türleri (%11.8) ve diğer Gram-negatif bakteriler (%8.6) oluşturmaktaydı. Etkenler arasında sefepime karşı direnç Acinetobacter türlerinde %44.8, Pseudomonas türlerinde %22.5, Klebsiella türlerinde %13.5 Enterobacter türlerinde %9.1 ve Escherichia coli' de %4.2 olarak saptandı.
